IL 60517 Phone: (630)-985-1300 Fax: (630)-985-1310 Relationship To The Original Contract and Purpose: This is an addendum to the existing WebOA Services Purchase Aareement executed between WEBQA, Inc. ( "WEBQA ") and City of Elgin, IL ("Customer") signed September 1.1, 2015 with its current term starting March 1, 2017 and ending February 28, 2018 ( "Current Contract Period"). The purpose of this Addendum is to add the new services selected below. Additional Services: Customer is ordering the following services at the following prices. Subscribed Services GovQA Employee Grievance Management System I Provides the ability to enter, track, and monitor employee grievances. WebQA Agrees To Provide the Following Functionality: • Initial entry of grievance via administration portal • The ability to distinguish between disciplinary and non - disciplinary grievance type • Notification of grievance entry to other select staff (Attorney) • Ability at first stage of process to act a due date and a reminder at the request level • If contested, the request owners in stages 2 -4 will have the ability to set due date reminders via Activities • At all stages, the ability to upload email communications, relevant documents, as well as add notes within the request • Notification to select Staff (Attorney) when grievance requests are closed and re- opened • A view for select Staff (Attorney) to see all grievances within the GovQA system and an automated grievance request report emailed to select Staff (Attorney) at desired frequency - Training and Ongoing Support: • One online training session. • Customer will log ALL ISSUES, including high - priority at www.suppoaga.com to receive service. Software Subscription Coats Software License(s) Annual Cost GovQA Employee Grievance Management System $600 Total Annual $600 One Time Implementation Costs GovQA Employee Grievance Management System Waived Total Waived Start Date and Tiutefratnes: The START DATE for the term of this agreement will be July 1, 2017 and go to a SYNC DATE of February 28, 2018 at which point the term for these Additional Services will sync to the term of the Original Contract and all subsequent renewals. Billing: Billing from START DATE to SYNC DATE Will occur on the signing of this Schedule B and include the monthly fees and implementation fee associated with the Additional Services described above. After the SYNC DATE, the billing period for these. Additional Services will be incorporated into the billing of other WebQA services to the customer consistent with the Original Contract and all subsequent renewals.
