RESOLUTION
APPROVING CHANGE ORDER NO. 1 IN THE CONTRACT
WITH BROITZMAN PROFESSIONAL CEMETERY SERVICES
WHEREAS, the City of Elgin has heretofore entered into a contract with Broitzman
Professional Cemetery Services for burial services at Bluff City Cemetery; and
WHEREAS,it is necessary and desirable to modify the terms of the contract a. is described
in Change Order No. 1 attached hereto.
NOW, THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF T E CITY OF
ELGIN,ILLINOIS,that Sean R, Stegall,City Manager,be and is hereby authorized a d directed to
execute Change Order No. 1,a copy of which is attached hereto and made a part hereo by reference.

PROJECT: CEMETERY CONTRACTUAL BURIALS
CHANGE ORDER: No. 1
CONTRACT: An Agreement dated November 10, 2006 with Broitzman Professional Cemetery
Services for burial services at Bluff City Cemetery for the period of January 1, 2007 through
2009. The service costs were originally quoted on Bid Number 06-117.
SCOPE: Request is to approve additional $10,000 in funds to Purchase Order# 238502 for buri-
al service costs through the end of December, 2009.
REASONS FOR CHANGE: The change is germane to the original contract as signed; and is in
the best interests of the City and is authorized by law.
SUMMARY OF CHANGES IN CONTRACT AMOUNT:

The original amount of the contract with Broitzman Professional Cemetery Services is $105,000.
Change Order No. 1 and the previous addition to the Purchase Order by the Purchasing Depart-
ment totaling $19,900 will increase the original contract amount by a total of$19,900, that sum
representing sixteen percent(16%) of the original contract amount. The new contract amount,
inclusive of all change orders is $124,900.
OTHER CONTRACT PROVISIONS: All other contract provisions remain the same.

SUBJECT: Change Order No. 1 with Broitzman Professional Cemetery Services for 2009
Burial Services
PURPOSE
The purpose of this memorandum is to provide the Mayor and members of the City Council with
information to consider approval of Change Order No. 1 to Broitzman Professional Cemetery
Services for additional burial services.
RECOMMENDATION
eIt is recommended that the City Council approve Change Order No. 1 with Broitzman
Professional Cemetery Services for burial services performed in 2009 in the amount of$10,000
for a 2009 contract value of$124,900.
BACKGROUND
On November 10, 2006 the City awarded Bid Number 06-117 to Broitzman Professional
Cemetery Services to perform burials at Bluff City Cemetery. The contract term was for three
years beginning January 1, 2007 through December 31, 2009. Included in the contract was an
option to renew the contract for two (2) one year terms which staff agrees to and Purchasing has
sent written notification to Broitzman of this decision. The value of the contract for 2009 was
$105,000 and burial services are based on a unit price (see attached contract). The total number
of burials to be performed in a given year is unknown, and in October 2009, an additional $9,900
was added to the purchase order for payment of services. The attached change order is to add
$10,000 also for payment of services for 2009 and will bring the 2009 contract amount to
$124,900.

FINANCIAL IMPACT
The total cost of Change Order No. 1 with Broitzman Professional Services is $10,000. Funds
are budgeted ($145,970) and available (17,800) in the 2009 Cemetery Fund, account number
294-3372-767-45.99,"Miscellaneous Services"to pay for the change order.

1. The City Council may choose to approve Change Order No. 1 with Broitzman
Professional Cemetery Services in the amount of$10,000.

registration fee for players because the increased cost would probably eliminate a number of
young people from being able to participate.
Clarence Hayward stated that since the city and the library have large billboards, he thinks the
Council should also allow the Little League teams to continue having billboards. He wished the
Mayor, City Council, and all citizens a happy holiday season.
Michael DeBrocke stated that the residents had been promised that a traffic signal would be
installed at the intersection of Shales Parkway and Summit Street. The State of Illinois states
that it is warranted,but they have no money. The City's budget no longer includes installation of
the traffic signal. However, he hopes that the City will proceed with installing the traffic signal.

contract for the dry cleaning of the Police and Code Uniforms in the amount of$10,775.75 to
Clock Tower Cleaners and Laundry and approve a contract for laundry service in the amount of
$19,548.60 to West Pier Landry for the Fire Department. Upon a roll call vote: Yeas:
Councilmembers Figueroa, Kaptain, Rodgers, Sandor, Walters and Mayor Schock. Nays: None.

Councilmember Kaptain made a motion, seconded by Councilmembtt Rodgers,to approve a
contract to Broitzman Professional Cemetery Service for a not to exceed amount of$105,000 per
year for burials at the Bluff City Cemetery at the unit prices noted in the memo provided to
Councilmembers. Upon a roll call vote: Yeas: Councilmembers Figueroa, Kaptain, Rodgers,
Sandor, Walters and Mayor Schock. Nays: None.
PUBLIC HEARING FOR THE 2007 PROPOSED CITY OF ELGIN BUDGET
Mayor Schock declared the public hearing open. Assistant City Manager Stegall stated that the
proposed budget for 2007 is in the amount of$296,842,960.
Clarence Hayward asked where the funding is coming from for the full-time animal control
officer. Mr. Stegall responded that it is coming from the Police Department funding. Mr.
Hayward stated that he was relieved that it isn't coming from his pocket.
No one else chose to speak. Mayor Schock declared the public hearing closed.
