RESOLUTION
APPROVING CHANGE ORDER NO. 1 AND FINAL IN THE CONTRACT WITH
MARTAM CONSTRUCTION, INC.
(Lawrence and McClure Water Main Project)
WHEREAS, the City of Elgin has heretofore entered into a contract with Martam
Construction, Inc. for the Water Main on Lawrence Avenue from State Street to Airlite Water
Treatment Plant and McClure from Lawrence to Murray Avenue Project; and
WHEREAS,it is necessary and desirable to modify the terms of the contract as is described
in Change Order No. 1 and Final, attached hereto.
NOW, THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF
ELGIN,ILLINOIS,that Olufemi Folarin,City Manager,be and is hereby authorized and directed to
execute Change Order No. 1 and Final,a copy of which is attached hereto and made a part hereof by
reference.

II . CHANGE ORDER CONDITIONS
1 . This Change Order does not relieve the Contractor form
strict compliance with the guarantee provisions of the
Original Contract, particularly those pertaining to
performance and operation of equipment .
2 . The Contractor expressly agrees that he will place
under his coverage of his Performance and Payment bonds

SUBJECT: Change Order No.1 and Final Acceptance of the Water Main on Lawrence
Avenue from State Street to Airlite Water Treatment Plant and McClure from
Lawrence to Murray Avenue project.
PURPOSE
The purpose of this memorandum is to provide the Mayor and members of the City Council with
information to approve Change Order No. 1, and final acceptance of the water main project on
Lawrence Avenue, from State Street to the Airlite Water Treatment Plant, completed by Martam
Construction.
RECOMMENDATION
rik It is recommended that the City Council approve Change Order No. 1 and the final acceptance
for the Lawrence Avenue from State Street to Airlite Water Treatment Plant and McClure from
Lawrence to Murray Avenue water main project and authorize the execution of final Change
Order No. 1 in the amount of$80,566.00 (deduct)with Martam Construction.
BACKGROUND
The Lawrence Avenue to Airlite water main project extended a 20-inch water main from the 24-
inch main at Lawrence and McClure Avenues to the Airlite Water Treatment Plant (WTP). The
project was approved by the City Council on May 12, 2004. This main transfers water from the
Riverside WTP to the Airlite reservoirs, where it can be re-pumped to the high and west pressure
zones.
Change Order No. 1 and Final covers 16 items, which includes 13 additional and 3 deducts. The
additional items are as follows:
16-inch pipe on Lawrence Avenue from Jackson to State Street $100,676.00
Two 8-inch services connections 11,200.00
18.1 ft of 36-inch storm sewer at Thomas More 5,506.12
6-inch sewer replacement at Hubbard Avenue 2,905.00
