RESOLUTION
ACCEPTING THE PROPOSAL OF TRUSTMARK INSURANCE COMPANY
FOR STOP LOSS AND AGGREGATE INSURANCE COVERAGE AND TPA SERVICES
IN CONNECTION WITH THE CITY OF ELGIN'S MEDICAL INSURANCE PPO PLAN
AND AUTHORIZING THE EXECUTION OF DOCUMENTS RELATED THERETO
BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF ELGIN,ILLINOIS,that
the City of Elgin hereby accepts the proposal of Trustmark Insurance Company for stop loss and
aggregate insurance coverage and TPA services in connection with the City of Elgin's medical
insurance PPO plan for the period of March 1, 2004 through February 28, 2005.
BE IT FURTHER RESOLVED that David M. Dorgan, City Manager, be and is hereby
authorized and directed to execute all documents necessary and incident to such proposal of
Trustmark Insurance Company.
