CHANGE ORDER NO. 1 AND FINAL FOR CONTRAC WITH MIDCO, INC.
SCOPE: Change Order No. 1 and Final will add one work it-m to the lump sum contract with
MidCo, Inc for the New Public Works Facility Door Security Project.
REASONS FOR CHANGE: This Change Order No. 1 and I inal is in the best interest of the
City and is authorized by law. The construction work scope :djustments were unforeseen at the
time of original contract award.
SUMMARY OF CHANGES IN CONTRACT AMOUN : The amount the contract will be
revised is as follows:

The original contract with MidCo, Inc. is for$39,910.22. B, reason of Change Order No. 1 and
Final,the contract for the New Public Works Facility Door 'ecurity Project will be increased by
the amount of one thousand, one hundred and sixty-four an. 82/100 dollars($1,164.82). The
new contract amount, inclusive of Change Order No. 1 and I inal, is forty-one thousand, seventy-
five and 04/100 dollars ($41,075.04).
OTHER CONTRACT PROVISIONS: All other contract isrovisions shall remain the same.
