CHANGE ORDER NO. 1 AND FINAL FOR BID ITEM D-3
SCOPE: Change Order No. 1 and Final will add one workItem to the lump sum contract with
Valley Fire Protection Systems, LLC for Bid Item D-3, "Fire Protection System"of the New
Public Works Facility Project - Bid Package D.
REASONS FOR CHANGE: This Change Order No. 1 and Final is in the best interest of the
City and is authorized by law. The construction work scope adjustments were unforeseen at the
time of original contract award.
SUMMARY OF CHANGES IN CONTRACT AMOUNi': The amount the contract will be
revised is as follows:
ITEM TOTAL
1. Furnish and install 4 sprinkler heads in employee entrance hallway $590.00
outside of locker room. Not shown on plans.
2. Furnish and install 7 sprinkler heads in extension of Trainig Room. $1,035.00
The original contract with Valley Fire Protection Systems, L C is for$26,000.00. By reason of
Change Order No. 1 and Final, the contract for Bid Item D- , "Fire Protection System"of the
New Public Works Facility Project - Bid Package D will be ' creased by the amount of one
thousand, six hundred and twenty-five and 00/100 dollars($ ,625.00). The new contract amount,
inclusive of Change Order No. 1 and Final, is twenty-seven t ousand, six hundred and twenty-five
and 00/100 dollars ($27,625.00).
OTHER CONTRACT PROVISIONS: All other contract provisions shall remain the same.
