CHANGE ORDER NO. 1 AND FINAL FOR BID ITEM'',C-9 & 10
SCOPE: Change Order No. 1 and Final will add four work items to the lump sum contract with
Johnson Floor Co., Inc. for Bid Items C-9 & 10, "Tile & Carpet" of the New Public Works
Facility Project - Bid Package C.
REASONS FOR CHANGE: This Change Order No. 1 and'Final is in the best interest of the
City and is authorized by law. The construction work scope adjustments were unforeseen at the
time of original contract award.
SUMMARY OF CHANGES IN CONTRACT AMOUNT: The amount the contract will be
revised is as follows:

1. Install tile and vinyl base for employee entrance hallway oiitside locker $662.00
room. Not shown on plans.
2. Material and labor to install three color pattern in lunch room, Snow $722.00
Command and employee entrance hallway in place of two color
pattern as specified.
3. Provide and install 34 square yards of carpet for extension'to Training $865.00
Room.
4. Provide balance of carpet dye lot for attic stock. $444.00
The original contract with Johnson Flooring Co., Inc. is for $70,039.00. By reason of Change
Order No. 1 and Final, the contract for Bid Items C-9& 10, `Tile & Carpet" of the New Public
Works Facility Project - Bid Package C will be increased byhe amount of two thousand, six
hundred and ninety-three and 00/100 dollars($2,693.00). Tiie new contract amount, inclusive of
Change Order No. 1 and Final, is seventy-two thousand, seven hundred and thirty-two and
00/100 dollars($72,732.00).
OTHER CONTRACT PROVISIONS: All other contract provisions shall remain the same.
