CHANGE ORDER NO. 1 AND FINAL FOR BID ITEM C-2
SCOPE: Change Order No. 1 and Final will add one work i em to and delete one work item
from the lump sum contract with PBM Concrete, Inc for Bid Item C-2, "Pre-cast Concrete"of
the New Public Works Facility Project - Bid Package C.
REASONS FOR CHANGE: This Change Order No. 1 and Final is in the best interest of the
City and is authorized by law. The construction work scope ,djustments were unforeseen at the
time of original contract award.
SUMMARY OF CHANGES IN CON RACT AMOUN : The amount the contract will be
revised is as follows:

1. Delete cutting existing pre-cast panels in Snow Command oom for -$1,135.00
new window.
2. Repair damage to existing pre-cast building wall panels do e by const- $1,394.00
equipment during site demolition work by others.
The original contract with PBM Concrete, Inc. is for$190,0 1.00. By reason of Change Order
No. 1 and Final, the contract for Bid Item C-2, "Pre-cast Co i crete"of the New Public Works
Facility Project - Bid Package C will be increased by the am•unt of two hundred, fifty-nine and
00/100 dollars ($259.00). The new contract amount, inclusi e of Change Order No. 1 and Final,
is one hundred and ninety thousand, three hundred and twen% and 00/100 dollars($190,320.00).
OTHER CONTRACT PROVISIONS: All other contract provisions shall remain the same.
