RESOLUTION
AUTHORIZING EXECUTION OF AN AGREEMENT WITH
UNICARE HEALTH PLANS
BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF ELGIN,
ILLINOIS, that Joyce A. Parker, City Manager, be and is hereby
authorized and directed to execute an agreement on behalf of the
City of Elgin with Unicare Health Plans for an employee health
insurance program, a copy of which is attached hereto and made a
part hereof by reference .

At the request of Mr. Kurt Schmitke at Global Benefits, I am summarizing the rates and benefit changes for
your renewal.Please have the appropriate party at the City of Elgin acknowledge these items and return to
my attention.Following are the applicable issues:
♦ Renewal date March 1,2001
♦ HMO rates effective March 1,2001:
Single: $173.08
Family: $475.96
• Addition of year-round part-time employees with 20 or more hours per week effective February 1,
2001. (part-time employees contribute 50%of the premium)
• Change of$5rx copay to$5 generic/$10 Brand/$25 Non-Formulary effective March 1,2001.
*Waiting for written request form City of Elgin to make this change.
