RESOLUTION
APPROVING CHANGE ORDER NO. 2 IN THE CONTRACT WITH
CERTIFIED MIDWEST CONSTRUCTION INCORPORATED FOR THE
HEMMENS ROOF/PAVING PROJECT
WHEREAS, the City of Elgin has heretofore entered into a
contract with Certified Midwest Construction Incorporated for
the Hemmens Roof/Paving Project; and
WHEREAS, it is necessary and desirable to modify the terms
of the contract as is described in Change Order No. 2, attached
hereto.
NOW, THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF THE
CITY OF ELGIN, ILLINOIS, that Joyce A. Parker, City Manager,
and Dolonna Mecum, City Clerk, be and are hereby authorized and
directed to execute Change Order No. 2 , a copy of which is
attached hereto and made a part hereof by reference.

CHANGE ORDER NO. 2

TRADE CONTRACTOR: Certified. Midwest Construction Inc.
Purchase Order No. 063632
SCOPE: This Change Order No. 2 will provide additional services
necessary to complete the Hemmens Roof/Paving Project .
It will amend the scope of work and add the following
items :
1 . Removal cf an aaditional layer of water proofing
membrane and sand bed.
2 . Removal of a section of sidewalk and the
replacement of same, including necessary curb and
handicapped ramp with matching colored concrete .
REASON FOR CHANGE:
1 . The removal of the layer of water proofing membrane and sand
bed was required because the original material was wet and
badly damaged. It was necessary to remove and replace the
material .
Cost $ 5, 306 . 00
2 . Removal and replacement of sidewalk sections and handicapped
ramp. It was necessary to remove the next section of sidewalk
north of the present contract limits, including the curbing
and handicapped ramp. The change was required to eliminate
the ponding of water at the joint between the new work and
existing sidewalk and to ensure that the handicapped ramp was
ADA compliant .

the time the contract with Certified Midwest Construction,
Inc . was authorizes.
• It is in the best interest of the City to approve the above
changes .
• These changes are germane to the intent and scope of the
original contract .
