RESOLUTION
AUTHORIZING EXECUTION OF AN AGREEMENT WITH HMO ILLINOIS
BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF ELGIN,
ILLINOIS, that Larry L. Rice, City Manager, be and is hereby
authorized and directed to execute a Revised Benefit Program
Application on behalf of the City of Elgin with HMO Illinois
for an employee health insurance program, a copy of which is
attached hereto and made a part hereof by reference.
