88-0727 GF Structures Corp ELGIN, ILLINOIS LORD'S PARK MIDDLE DAM CHANGE ORDER NO. 1 SCOPE: This Change Order No . 1 covers the additional concrete required to complete the side walls of the middle dam underneath the foot bridge at Lord ' s Park . The general contractor operating under contract to the City of Elgin is G. F. Structures Corp . of Dundee, Illinois 60118-0279. REASONS FOR CHANGE: This change order is required in order to complete the proposed improvement at the middle dam site. Engineering staff noticed that after the site was dredged , the existing flagstone wall conditions differed from the prepared plan. Instead of a straight vertical wall , the bottom of the wall extended out at the base to serve as a footing . Since there were no original construction plans for the site, this condition was discovered after the excavation at the site. In addition, the condition of the flagstone wall was poor and any breaking off or cutting of the protruding flagstone could result in the wall collapsing . Therefore, the design was altered to compensate for a wider base of the 10" concrete wall . The new design provides a slanted wall extending out at the base and tapering in towards the top of the wall . The contractor , G. F. Structures, proposed that this work would be done at a cost of the additional concrete to complete this work.. SUMMARY OF CHANGES IN CONTRACT AMOUNT: The amount of the contract will be revised as follows: ITEM ADD 10" Concrete Walls; Cubic Yard $4,699.50 7.23 cubic yards a $650.00 DEDUCT Dowels Across Cracks $ 40.00 2 each a $20.00 The present contract with G. F. Structures Corp . is for $59,597.50. By reason of this Change Order No . 1 , the contract for the Lord 's Park Middle Dam will be increased in the amount of Four Thousand Six Hundred Fifty-Nine and 50/100 Dollars ($4,659.50) , the total contract value being Sixty-Four Thousand Two Hundred Fifty-Seven and 00/100 Dollars ($64,257.00) . BACKGROUND: The City Council awarded the Lord' s Park Middle Dam Rehabilitation project to G.F. Structures at the Council meeting on September 9 , 1987 . This Change Order is required in order to complete the proposed improvement at the middle dam site. Engineering staff noticed that after the site was dredged, the existing flagstone wall conditions differed from the prepared plan. The work for the rehabilitation of the middle dam has already been completed. If the additional quantity of concrete for the new walls was not used, the project would not have been able to be completed. FINANCIAL IMPACT: The original awarded contract was in the amount of $59 ,597 . 50 . By reason of this Change Order No. 1 , the contract will be increased in the amount of $4 ,659 .50 , for a total estimated cost of $64 ,257 .00 . The additional funds to cover the cost of this change order can come from the savings 16 ,370 .00) resulted from the Lord' s Park Lagoon Dredging projtect which was completed at a cost of 22% under the budgeted amount of $75 ,000 .00 . A copy of the Change Order is attached for your consideration. BACKGROUND: As earlier presented to the City Council in execu- tive session on April 26 , 1988 , there were concerns in regards to the quality of some concrete work. These concerns were expressed to the contractor along with a request to correct the situation. The contractor, G.F. Structures, hired a private testing company which took five core samples from the concrete wall which had severe honeycombing. The contractor met with City staff on May 4 , 1988 and presented the results of the tests for review. These tests indicated that the concrete wall was not constructed as specified. The test results revealed that the honeycombing (lack of consolidation) extended through the outer six (6) inches of the wall. Also, it was revealed that the honeycombing was along some of the reinforcement steel and that some of the steel, both grids , were located in the outer six inches of the wall. Repair of the honeycombing will not produce a satisfacto- ry wall. G.F. Structures was notified that they are required to remove and replace the wall . Also, a new footing shall be constructed on top of the existing footing. This new footing shall be constructed as specified on the project plans. The new footings are complete , construction will progress to the con- crete walls. FINANCIAL IMPACT: Once the work which was included on the first pay estimate is completed and approved, we shall issue the first payment to G.F. Structures. All other payments to the contrac- tor will be made as satisfactory work has been completed. CONCLUSION: Now that work has continued on the project, there will be increased inspection on the job and all completed work will be approved prior to progressing with the next stage . April 27 , 1988 MEMORANDUM TO: Mayor and Members of the City Council FROM: City Manager SUBJECT: Lord' s Park Middle Dam Renovations PURPOSE: This memorandum will provide the Mayor and Council with an update on the construction delays with the subject project. BACKGROUND: G. F. Structures Corporation of Dundee, Illinois, is the contractor performing the renovations at the Lord' s Park middle dam. This work mainly involves the pouring of new steel reinforced concrete footings , walls and wingwalls . This project is located between the upper and lower lagoons at the park and will be affected constantly by the water flow between the lagoons. On April 11 , 1988 , the City staff became aware that the rein- forcement steel apparently had not been installed as specified in the plans for the project. During an indepth inspection, it was discovered that the specified reinforcement steel was not used and that the one concrete wall that was poured had severe honeycombing. This honeycomb condition resulted from the lack of use of a concrete vibrator during the concrete pour. The vibrator agitates the concrete and minimizes the voids in the concrete. Upon seeing these unconformities with the project, work was stopped and a meeting was called with G.F. Structures to discuss the construction conditions . Members of G.F. Structures met with City staff on April 12 , 1988 , and stated that all reinforc- ing steel was properly located, but one of the specified bar types was not of the proper width. The staff requested the contractor to remove a section of the poured footing to expose the steel so we could determine if the steel was properly located and of the type specified and the contractor agreed. After one week of not breaking open the footing, the contractor again met with City staff on April 18 , 1988 and stated that the bars used were not the correct ones as previously stated. It was decided at this meeting that the design engineer from Donohue and Associates would be contacted with respect to developing an adjusted wall and footing design which shall meet the requirements of the project. G.F. Structures would hire their own structural engineer and testing services to complete the study of the existing wall and footings. Mayor and Members of the City Council April 27 , 1988 Page 2 A private testing company was onsite on April 22 , 1988 to take five core samples from the concrete wall. These samples will be inspected and tested to determine the condition of the wall and a report will be made available to the City and Donohue and Associates for review. FINANCIAL IMPACT: The first payment to the contractor for this project has been processed, but the payment is being held until the unconformities of this project are rectified to the satis- faction of the Engineering Division. CONCLUSION: It is the opinion of the staff that the poured concrete wall be removed because of the servere honeycombing and improper placement of the reinforcing steel which we feel will be verified by the test work. The condition of the footings and any recommended design changes will be reviewed by the staff and implemented as necessary.
