WHEREAS, the City desires to assist in the implementation of a program providing preventive health care for children and adolescents from low income families in the City of Elgin. WHEREAS, the Well Child Conference is a program which provides preventive health care for children and adolescents in the City of Elgin. It is hereby agreed: 1 . The Well Child Conference will provide preventive health care services for children and adolescents from low income families . The provision of preventive health care services shall take place at 5 North Geneva Street. 2 . The Well Child Conference will rehabilitate and equip a building area of approximately 1,600 square feet in a manner which is suitable for providing health care services and use as an indoor children' s recreation area, and parenting learning center. 3 . The Well Child Conference shall submit a report which documents all work completed regarding this rehabilitation, all equipment purchased and all sources of revenue. 4 . The City shall provide funds to the Well Child Conference in an amount not to exceed $32, 100 for the express purpose of reimbursing the Well Child Conference for funds expended for the rehabilitation of building space and the purchase of equipment for the property located at 5 North Geneva Street, upon presentment by the Well Child Conference of the written report described in paragraph 3, the contents of which are found to be satisfactory to the City. Rice, City Manager SUBJECT: Purchase of Service Agreement - Well Child Conference Purpose: To consider a Purchase of Service Agreement with the Well Child Conference organization. Background: As part of the Community Development Block Grant hearing process the allocation of General Fund dollars to certain r organizations was considered and approved by the City Council. The Well Child Conference was allocated $ 32,100. for the costs of rehabilitating building space and providing equipment for the renovated space located at 5 North Geneva Street. The Well Child Conference has a lease for the subject premises through the year 2002. Financial Impact: The $32,100 allocation was made from a total of available General Fund dollars of $560 ,000. Therefore sufficient funds exist in the budget for this activity. Recommendation: It is recommended that the City Council approve the attached Purchase of Service Agreement. A copy has been forwarded to the Well Child Conference organization. If approved a resolution will be prepared for action at the next City Council meeting. dl. Larry L. , City Manager f August 19 , 1987 MEMORANDUM TO: Mayor and Members of the City Council FROM: City Manager SUBJECT: Change Order No. 1 for the Well Child Conference Rehabilitation PURPOSE: To consider a proposed change order for the Well Child Conference rehabilitation. BACKGROUND: The 1986-87 Community Development Block Grant Program allocated $30 , 000 towards the rehabilitation of the lower level of the Masonic Temple for use by the Well Child Conference. The actual bid award totaled $30 ,289 . The remainder of the rehabilitation work was to be funded by the Well Child Conference ( similar to the Old Main and Community Crisis Center projects ) . For both Old Main and the Community Crisis Center projects , change orders were the responsibility of the building owner or lessee, i.e. the organization receiving the grant funds. The change order requested for the Well Child Conference totals $900 and is requested in order to provide baseboards . The original rehabilitation specifications inadvertently omitted baseboards as part of the scope of work. FINANCIAL IMPACT: The bid award for this project totaled $30 ,289 . The proposed change order is $900 . Sufficient CDBG contingency funds remain to cover the proposed change order. RECOMMENDATION: It is recommended that the City Council consider the proposed change order and direct staff to take appropriate action. 41111" , es J. BACKGROUND: In January, the City Council authorized the staff to participate in the Well Child Confere 's second annual Clean-A-Thon. The wor ay project was held on May 2nd with approximately 500 volunteers involved in various clean up activities. This year' s clean up projects featured removing debris and brush from the Bluff Spring Fen, Walton Island, Lower Wellington, park stream beds and the River Trail. In addition, park picnic shelters were painted, graffiti was removed at several locations and fire hydrants were painted. Photographs taken during the clean up activities are attached. The Clean-A-Thon project has significantly enhanced the appearance of the community. Moreover, because of the extensive publicity surrounding the Clean-A-Thon, three other groups have completed smaller but nevertheless worthwhile clean up projects for the City. FINANCIAL IMPACT: The cost to the City of participating in the Clean-A-Thon was $2 ,040 , well below the original estimate of $6, 500. Improved planning and coordination based on last year's experience greatly reduced the involvement of City crews, thereby lowering operational costs. In addition, because of the inclement weather, fewer fire hydrants were painted than originally anticipated, decreasing the amount of supplies used. CONCLUSION: The Clean-A-Thon proved to be an excellent community beautification project as well as a successful fundraiser for the Well Child Conference. The Well Child Conference has expressed interest in continuing the Clean-A-Thon next year ( see attached letter) . 4W1 ames J.
