Crack Sealing
Dear Mr . Chaudhry:
I would like to apologize for my oversight and request
a contract change to the subject contract at this time. The
undersigned has determined that the circumstances which
necessitate this change were not foreseeable at the time the
contract was signed .
The change was a result of over estimating the lineal
footage of cracks on a majority of the streets listed in the -
contract . All streets listed were sufficiently completed by
the contractor . Attached is a copy of the completed BLR 6301
form. I believe you have the contractor ' s final invoice and
waiver .
Please contact the undersigned if additional
documentation needs to be provided . The undersigned may be
