RESOLUTION
AUTHORIZING EXECUTION OF AN AGREEMENT FOR SPECIAL RECREATION WITH
THE JAYNE SHOVER EASTER SEAL REHABILITATION CENTER,
BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF ELGIN, ILLINOIS,
that Richard L. Verbic, Mayor, and Marie Yearman, City Clerk, be and are hereby
authorized and directed to execute an agreement on behalf of the City of Elgin with the
Jayne Shover Easter Seal Rehabilitation Center for special recreation, a copy of which is
attached hereto and made a part hereof by reference.

SERVICE PURCHASE AGREEMENT BETWEEN
JAYNE SHOVER EASTER SEAL REHABILITATION CENTER, INC.
AND
THE CITY OF ELGIN
Agreement between the Jayne Shover Easter Seal Rehabilitation Center, Inc. ,
hereinafter referred to as Center, and the City of Elgin, hereinafter re-
ferred to as City, Witnesseth:
1. The Center agrees to provide special recreation programs for handicapped
residents of the City of Elgin under the direction of qualified recreation
specialists. The special recreation programs offered by the Center are
open to all disabilities as well as all ages.
2. The Center offers the following sixteen (16) recreation programs during
the Spring, Summer, and Fall 1986, and Winter 1987 sessions. Transport-
ation will be provided for some of the programs.

Aerobic Dance Learning Disabled, Educable Mentally Handi-
capped, Trainable Mentally Handicapped, Be-
havior Disordered, Visually Impaired, Hearing
Impaired, Physically Handicapped, Emotionally
Disturbed, Speech and Language Delayed/Dis-
ordered.
Adaptive Exercise Physically Disabled
Bowling for Fun Learning Disabled, Educable Mentally Handi-
capped, Trainable Mentally Handicapped, Be-
havior Disordered, Visually Impaired, Hearing
Impaired, Physically Handicapped, Emotionally
Disturbed, Speech and Language Delayed/Dis-
ordered.
Canoeing Physically Disabled
Cooking Learning Disabled, Educable Mentally Handi-
capped, Trainable Mentally Handicapped
Fishing Learning Disabled, Educable Mentally Handi-
capped, Trainable Mentally Handicapped, Be-
havior Disordered, Visually Impaired, Hear-
ing Impaired, Physically Handicapped, Emo-
tionally Disturbed, Speech and Language
Delayed/Disordered.

3. The Center agrees to voucher the City of Elgin for a selection from the above
named programs at a total cost not to exceed $10,000.00 and agrees to voucher
the City of Elgin for only participants who are residents of the City of Elgin.
4. The Center agrees to appropriately name the City of Elgin as the funding source
for the above mentioned programs in all materials distributed in the area, i.e. ,
brochures, radio spots, news releases, etc.

5. The Center will voucher the City of Elgin per program, per session at the
end of each session based on the costs shown on each program worksheet.
However, the billing will be adjusted based on actual enrollment,
variable facility rental cost, and actual percentage of Elgin residents
in each program.
6. The agreement may be cancelled by either party hereto at any time with
sixty (60) days written notice by delivering personally or by U.S. Mail
said notice to the Center at 799 South McLean Blvd. , Elgin, Illinois,
or to the City, 150 Dexter Court, Elgin, Illinois.
7. The Center does hereby, for itself, its heirs, personal representatives,
and assigns, covenant and agrees with the City of Elgin to forever fully
protect, defend, and save harmless the City of Elgin, Elgin, Illinois,
from any and all lawsuits, costs, damages, attorney's fees and expenses
of every kind and nature which it may suffer, expend, or incur under or
by reason, or in consequence of the Center providing special recreation
programs for the City of Elgin and to defend, at its own costs and charges
on behalf of and for the protection of the said City of Elgin, Elgin,
Illinois, any legal action which may be instituted as a consequence of the
Center providing special recreation programs.
