Police/Provider Working Agreement
The agreement between the City of Elgin and Centro De Informacion Y Progreso of Elgin
is enterd into divert youth from the juvenile justice and child welfare systems through the
use of community-based services.
*As long as funding is availabel from the Illinois Department of Children and Family
Services, Centro de Informacion agrees to provide youth Counseling and services

for uP to eight months to juveniles 12-17yrs. 2 arrests or who are referred
(Enter time Services will be Offerd) (enter Target Population) adjudicated delinquen
by a youth officer of the Elgin Police Department Crisis intervention

services will be renderd by Centro de Informacion within 120 minutes of the time

A call is received by Centro De Informacion .Otherwise referrals will be made

agrees to refer youth in the target population when there is evidence that the available
services will deter further involvement of the youth juvenile child welfare systems.
If either party become dissatisfide with the referral process, services renderd or
feedback offer d,the two a ree ,ta meet to reconcile differences. / I
