OTTER CREEK DETENTION AREA
Change Order No. 1
SCOPE. This Change Order No. 1 shall cover the installation of 37. 5
Lineal Feet of 60 inch Diameter Class 111 Concrete Pipe,
which is an extention of the existing 60 inch storm sewer
under U. S. 20 to the Otter Creek Detention Facility.
REASON FOR CHANGE. This section of the storm sewer, the extention of
an existing 60 inch storm sewer to the detention facility was included
in the original plans. The plans were sent to Illinois Department of
Transportation (IDOT) , Division of Highways and Division of Water Re-
sources (DOWR) for review several times. In one of the review processes,
this item was left out inadvertently from schedule of prices. This
omission was not detected in the in-house review.
CHANGE IN THE CONTRACT AMOUNT
Additional cost for 37.5 feet of 60 inch storm sewer
installed complete $5,800.00
By reason of this Change Order No. 1 , the contract shall be increased
in the amount of Five Thousand Eight Hundred and No/100 ($5,800.00) ,
the total contract cost with T & W Edmeir Corporation being Six Hundred
Fifteen Thousand Eight Hundred Seventy Six and 88/100 Dollars ($615,876.88) .
OTHER CONTRACT PROVISION. All other contract provision shall remain the
same.

TO: Bob Malm, Acting City Manager

SUBJECT: Otter Creek Detention Area
Attached is Change Order No. 1 for Otter Creek Detention Area.
The construction of the facility cannot be completed without extending
the existing storm sewer under U. S. 20.
This item was shown in the original plans. The plans were sent
several times to both Division of Highways and Division of Water Resources.
A number of changes were made by both agencies each time the plans were
reviewed.
In one of these review processes this item was eliminated by some one.
Unfortunately we did not detect that it was left out from the Summary of
quantities and Schedule of prices.
We request City Council approval for T & W Edmeir Corp' to proceed
with the installation.

Gentlemen:
Reference is madeto the above project and the job requirement
which necessitates removal of the existing 60" Diameter Flared End
Section and the addition of 37.5 Linial Feet of 60" Diameter Class
III Concrete Pipe.

We will remove the 60" Diameter Flared End Section and provide
and place the additional 60" Diameter Class III Concrete Pipe for the
Lump Sum Add of Five Thousand, Eight Hundred Dollars ($5,800.00) to our
base contract.
Please advise of your acceptance. I remain
