EASTER SEAL REHABILITATION CENTER, INC. AND THE CITY OF ELGIN
Agreement between the Jayne Shover Easter Seal Rehabilitation Center, Inc. ,
hereinafter referred to as Center, and the City of Elgin, hereinafter referred
to as City, Witnesseth:
1. The Center agrees to provide special recreation programs for handicapped
residents of the City of Elgin under the direction of a qualified rec-
reational therapist with a degree in Therapeutic Recreation. The special
recreation programs offered by the Center are open to all disabilities as
well as all ages.
2. The Center agrees to provide the following thirteen (13) eight week rec-
reation programs four times per year for the Spring 1985, Summer 1985,
Fall 1985, and Winter 1985 sessions. Transportation will be provided for
some of the programs.

3. The Center agrees to voucher the City of Elgin for the above named programs
at a total cost not to exceed 10,080.47 and agrees to voucher the City of
Elgin for only participants who are residents of the City of Elgin.
4. The Center agrees to appropriately name the City of Elgin as the funding
source for the above mentioned programs in all materials distributed in
the area ; i .e. , brochures, radio spots, news releases, etc.
5. The Center will voucher the City a maximum of $672.03 per program, per
session, based on 80% of the cost of the program. Previous enrollments
have proven that at least 80% of the participants of the recreation program
have been residents of the City of Elgin. The Center will voucher the
City at the end of each session based on the actual percentage of Elgin
residents in the program, not to exceed 80% of the cost of the program.
6. This agreement may be cancelled by either party hereto at any time with
sixty (60) days written notice by delivering personally or by U.S. Mail
said notice to the Center at 799 South McLean Blvd. , Elgin, Illinois, or
to the City, 150 Dexter Court, Elgin, Illinois.
7. The Center does hereby, for itself, its heirs, personal representatives,
and assigns, covenant and agrees with the City of Elgin to forever fully
protect, defend, and save harmless the City of Elgin, Elgin, Illinois,
from any and all lawsuits, costs , damages, attorneys ' fees and expenses
of every kind and nature which it may suffer, expend, or incur under or
by reason, or in consequence of the Center providing special recreation
programs for the City of Elgin and to defend, at its own costs and charges
on behalf of and for the protection of the said City of Elgin, Elgin,
Illinois, any legal action which may be instituted as a consequence of
the Center providing special recreation programs.

PROGRAM WORKSHEET SUMMARY
The cost for the programs listed on the program worksheets were calculated
in the following manner.
The cost for personnel was calculated per meeting and then multiplied
by the number of class meetings per 8 week session.
The cost for material required was calculated per meeting and then
multiplied by the number of class meetings per session.
The facility rental fee was calculated per meeting (when applicable)
and multiplied by the number of class meetings per session.
The total cost to run the program for 8 weeks was determined by adding
up the costs for personnel , material and facility rental . Since the est-
imated percentage of Elgin residents is 80%, the total cost of running the
program was multiplied by 80% to determine the cost of the program for Elgin
residents only.
The fees that Elgin participants pay in order to participate in the
program were deducted from the total program cost for Elgin residents.
This figure indicates the Easter Seal Recreation Department expense to
run the program for Elgin residents for one 8 week session. To determine
the total expense for one year, this figure was multiplied by the number
of sessions that the program is offered during a one year period.
The costs listed for personnel and material include not only the direct
costs of program staff, transportation and supplies, but the indirect costs
of administrative expenses, insurance, printing, and utilities.
The programs listed on the worksheets have proven to be very popular
during past sessions and registration for these programs has been successful .
However, in the event that any of the programs do not meet the minimum
registration requirement, alternative recreational programs are available.
The following programs have been offered in the past and have been successful .
These programs will be offered continually, some on a seasonal basis.
