FOR THE ALFRED/DFMIOND STREET IMPROVEMENTS WHEREAS, the City of Elgin has heretofore entered into a contract with E.M. Melahn for the Alfred/Demmond Street improvements; and WHEREAS, it is necessary and desirable to modify the terms of the contract as is described in Change Order Number 1, attached hereto. NOW, THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF ELGIN, ILLINOIS, that Leo Nelson, City Manager, be and is hereby author- ized and directed to execute Change Order Number 1, a copy of which is attached hereto and made a part hereof by reference. kichard L. This Change Order No. 1 covers the addition of the lowering of certain sanitary sewer stubs within the project. This work was done immediately to expedite the project. REASON FOR CHANGE. The new storm sewer which is to be installed along the westside of Alfred Street is at a depth of approximately five (5) feet. The sanitary sewer is at a depth of eight (8) to nine (9) feet in the center of the street. Four (4) of the sewer stubs to the west were found to have been installed at varying depth which put them in the path of the storm sewer. It was necessary to remove the old services and replace them from the property line to the main sewer with plastic sewer pipe. CHANGE IN CONTRACT AMOUNT: Time and material cost of lowering $5,411 .78 four (4) Sanitary Sewer Stubs By reason of this Change Order No. 1 , the contract cost shall be increased in the amount of Five Thousand Four Hundred Eleven and 78/100 ($5,411.78). OTHER CONTRACT PROVISIONS. All other provisions of the contract shall remain the same. Agreed to this day of , 1983. Ridley, Civil Engineer I(-k74 RE: Alfred/Demmond Street Improvements Change Order No. 1 While installing the storm sewer for the above referenced project, the contractor found that the sanitary sewer stubs for several of the houses had been raised above the grade they were supposed to be according to our plans. It was therefore necessary to have the services lowered so that the storm sewer could be installed to drain the Alfred and Demmond Intersection. The contractor was directed to lower the sewer services immediately to expedite the project. The work was to be done on a time and material basis plus ten percent (10%) . The invoice from Illinois Hydraulic (a sub-contractor) itemizing the work and a Invoice from E.M. Melahn Construction are included. The original engineers estimate was $171 ,846.25. The bid price awarded by the City Council on July 11 , 1983 was $126,836.45. The amount of increase in the contract price is $5,411 .78 which will make the new contract price $132,248.23. We would therefore request that Change Order No. 1 (copy attached) be approved by the City Council .
