ELGIN, ILLINOIS COMMUTER PARKING FACILITIES NATIONAL STREET COMMUTER PARKING LOT CHANGE ORDER NO.1 SCOPE. This Change Order No.1 covers additions and modifications to the above contract as follows: Item 1. The bank on the west side of the parking lot shall be excavated to a slope of 2:1 from stations 3+50 to 5+0. Included in this item will be the deletion of the proposed landscaping from stations 0+20 to 6+15 on the west side of the parking lot and the substituion of Class III seeding, fertilizer, and paper matting to retain the steep slopes. Item 2. The seven parking lot lights shall be relocated to the west side of the parking lot as directed by the engineer. The light bases shall be extended to a height of 18 inches above the back of curb. Item 3. The contractor was directed to install B6.18 curb and gutter along Walnut Avenue. Item 4. The contractor shall install seven (7) inch P.C.C. drive- way pavement through the approach area of the sidewalk at the entrance to the parking lot. Item 5. The contractor was directed to install a four (4) inch perforated underdrain from stations 3+0 to 4+50 approximately eighteen (18) inches east of the west curb. This work included the connection to the new catch basin at station 3+0. REASON FOR CHANGE Item 1. After construction began it was found that the bank on the West side of the parking lot was higher and closer to the parking lot than shown on the plans. The original plan called for a 6:1 slope, but this type of slope would have taken excavation to the C.&N.W. Railroad tracks west of the project. In order to keep the work within the original project limits, our office directed the contractor to excavate the bank to a 2:1 slope. The Landscaping called for on the west bank from stations 0+20 to 6+15 is Day Lillies with wood bark mulch. In conversations with two landscaping firms and the Parks Department a slope of 6:1 is the maximum slope that the above type of landscaping can be expected to maintain. A 2:1 slope requires seeding and matting. Item 2. The RTA requested that we move the parking lot lights from the east side of the parking lot to the west side because of the lighting plans they have for their platform. Because of the slope and the underdrain on the west side we kept the lights closer to the back of the curb. In order to protect the poles the contractor was directed to extend the foundations to eighteen (18) inches above finished grade. CO-1 Item 3. The drawings indicated a B6.12 curb & gutter along Walnut Avenue. Excavation along the street found B6.18 curb & gutter. To provide continuity and to assure the integrity of the street the contractor was directed to place B6.18 curb & gutter along Walnut Avenue. Item 4. The plan called for four (4) inch P.C.C. sidewalk throughout the project, including the drive approach to the lot from Walnut Avenue. It has been our experience that an approach thickness of six (6) to seven (7) inches is much more able to stand up to the traffic we expect to experience in the parking lot. Item 5. Because of wet subsoil there was water flowing over and under the curb on the west side of the parking lot from stations 3+0 to 4+50. This condition made it extremely difficult to guarantee that the pavement in that area would stand up to traffic. The solution to the problem was to install a four (4) inch under- drain eighteen (18) inches east of the curb and eighteen (18) inches below finish grade. SUMMARY OF CHANGES IN CONTRACT AMOUNT. The contract amount shall be revised on the following basis: ADD Item 1 . Additional costs relating to work on bank west of parking lot: Special Excavation 593 C.Y. @ $10.00 $5,930.00 Deleted Landscaping (6,699.60) New Landscaping 6,116.00 Total $5,346.40 $5,346.40 Item 2. We therefore have directed the contractor to make certain changes in the work to expedite completion of the project. Our office has prepared a Change Order No. 1 to cover all of the necessary extra work that explains in detail the changes and the reasons for them (copy attached). We request that the Council approve this Change Order No.1 in the amount of $7,862.90. Box 50194 Cicero, IL 60650 Re: National Street Commuter Parking Lot Dear Joe: Your Company, with the exception of four (4) days of work done by three (3) subcontractors, has not worked on the above referenced project since May 29, 1983, when the asphalt paving was completed. It is our intent that the parking lot be ready for use by July 15, 1983. You have only to replace the damaged portion of the concrete approach. The land- scaping and broken curb can be completed while cars are parking in the lot. • The asphalt paving installed on May 29, 1983,, is unacceptable in two respects. First, there are several roller marks in the pavement, pre- dominately in the driving lane. Our inspector instructed your personnel at the site to remove the marks by allowing the mat to cool and then finish rolling them. These instructions were ignored and the marks remain. Secondly, the pavement in several areas has failed to pass the minimum required compaction of 93%. In order to confirm or negate the compaction failures our office will require four (4) random cores from each of the three (3) areas shown on the attached drawing. The cores obtained from this work should be made within two weeks of your receipt of this letter and delivered immediately to our office. This work shall be performed at no extra cost to the City of Elgin, as outlined in Article 406. 15 of the "Standard Specifications. . . ". Our office met with you in late May, 1983 to discuss payment number two (2) . We have completed our payment estimate and await the following • items: 1 . An invoice itemizing the quantities discussed. . 2. Receipts for one (1 ) load of Bituminous Mixture and one (1 ) load of prime. 3. Sworn Statement. 4. Waivers from all subcontractors , suppliers and your company. Mr. Joe Serpico June 29, 1983 Page 2 Finally, the contract requires that your company and all sub- contractors working on this project submit a "Weekly Statement of Compliance". We have received several unsigned copies of this report from your company, but we do not have all of these reports. We have no statements from any of your subcontractors. We will require these forms immediately in order to meet the conditions of the contract documents as set forth by U.S.D.O.T. , I .D.O.T. , and the City of Elgin.
