RESOLUTION
AUTHORIZING STATEMENT OF WORK FROM DAMS & ASSOCIATES, INC. FOR
CITY MANAGER EVALUATION AND 2024 EVALUATION TOOL
BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF ELGIN, ILLINOIS,
that Richard G. Kozal, City Manager, be and is hereby authorized and directed to accept a
statement of work for city manager evaluation and 2024 evaluation tool from Dams&Associates,
Inc., pursuant to the proposal therefore dated August 27, 2024, a copy of which is attached hereto
and made a part hereof by reference.

SUBJECT: Statement of Work for Completing the 2023 City Manager
Evaluation and Preparing the 2024 Evaluation Instrument
Dear Carol and John:
Thank you for your invitation to submit this Statement of Work for
completing the 2023 City Manager evaluation by providing a report of the
results and debriefing the results with Council and Mr. Kozal.
This Statement is also a proposal to include modifications to the evaluation
instrument so the 2024 evaluation will be ready to go in January 2025.
I trust you will find this SOW complete and satisfactory. Please consider it a
starting point we can tweak to meet specific requirements.
In the meantime, should you have any questions or would like additional
information, please do not hesitate to call.

City of Elgin Statement of Work: 2023 City Manager Evaluation and 2024 Evaluation Tool
Activities and Deliverables Investment
1. Prepare and submit a report containing the results of the 2023 $1,350
City Manager evaluation. ITS will provide the raw data and their
calculations to our office. We will conduct our own computations
and solve discrepancies with ITS, if any.
Responses from these stakeholders will be included:
a. Council
b. Key staff
c. Key CMO staff
d. External stakeholders
e. Mr. Kozal's self-evaluation
These stakeholder categories are the same as for the 2022
City Manager evaluation.
f. In addition to the average results per survey item, we will also
report the range of responses as was requested by Council
during the debriefing of the 2022 evaluation.
2. Meet with Council in person to debrief the 2023 evaluation results $ 1,550
and evaluation tool. Includes all travel costs. No overnight.
a. During this debriefing, we will identify items in the survey that
require follow-up with the City Manager. We will also identify
potential changes council member may want to make for the
2024 evaluation instrument.
b. I will forward my notes of the meeting to Mayor Kaptain for
distribution to Council.
3. Prepare a draft of the 2024 draft instrument. $450
4. Meet with Council in person or via Zoom for facilitating the In person: $1,550
conversation between Council and the City Manager regarding Via Zoom: $850
items to be discussed (see Item 2) and review a draft of the 2024
evaluation instrument.
a. I will forward my notes of the meeting and the updated 2024
evaluation tool to Mayor Kaptain for distribution to Council.
