RESOLUTION
PROVIDING FOR TERMINATION OF AN AGREEMENT WITH AZAVAR AUDIT
SOLUTIONS, INC. FOR UTILITY BILL AUDIT SERVICES AWARDED AND PURSUANT
TO REQUEST FOR PROPOSAL NO. 12-048B UTILITY BILL AUDIT SERVICES
BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF ELGIN,ILLINOIS,that
City of Elgin hereby terminates the Agreement between the City of Elgin and Azavar Audit
Solutions,Inc.,dated November 6,2013,awarded in connection with request for proposal No. 12-
048B pursuant to Section 6 of such Agreement.
BE IT FURTHER RESOLVED BY THE CITY COUNCIL OF THE CITY OF ELGIN,
ILLINOIS,that Richard G.Kozal,City Manager,be and is hereby authorized and directed to send to
Azavar Audit Solutions,Inc.,a thirty(30)day written notice of such termination pursuant to Section
6 of the Agreement, and a rejection of the purported findings by Azavar Audit Solutions, Inc.
regarding the City's vehicle/motor fuel tax.
