L the undersigned, hereby certify that I am the sponsor or appointed representative of the organization requesting use of the
school district facility. Anyone knowingly or unknowingly misrepresenting the organization requesting use of the school
district facilities will be held liable for all facility rental costs. I hereby expressly agree on behalf of said organization to
indemnify and forever hold harmless School District U46, its officers, board members, and employees, individually and
collectively, as to any claim or claims which might arise out of the use of the school facilities as herein provided.
In addition, I will comply with the following:
1. Only that part of the building for which this request has been made shall be used. Special permission must be
obtained to use any other space.
2. Proper care will be given to the building and contents therein. I assume financial responsibility, individually and on
behalf of said organization, for that part of the school, or contents utilized therein, that might be damaged or stolen.
Representatives from other public agencies (park districts, village governments, etc.) will not be held personally
liable for their organization's rental events. Any damage or theft must be reported to School District personnel or
the Facility Rental Program office as soon as possible.
3. I agree individually and on behalf of said organization to meet any financial obligations for building rental and/or
custodial services that might be required by the Board of Education of School District U46.

4. 1 have attached to this contract a Certificate of Insurance providing liability coverage for the above organization
naming School District U46 as an additional insured as to the proposed amounts not less than the following:
1. General Liability:
1. Bodily Injury Liability of $100,000/$300,000 each occurrence, with an aggregate of
$1,000,000
2. Property Damage Liability of $250,000 each occurrence, with an aggregate of $250,000
3. Bodily Injury and Property Damage Liability combined of $1,000,000 each
occurrence, and $1,000,000 aggregate
2. Automobile Liability, if applicable:
1. Bodily injury each person of $250,000
2. Bodily injury each accident of $500,000
3. Property damage of $100,000
5. The adult in charge of the activity will be present during the entire rental period and any set up or break down. This
person will be responsible for the proper use of the facilities and the proper conduct of those in attendance. The
renter is recommended to have someone certified/trained in Cardiopulmonary Resuscitation (CPR), Automated
External Defibrillator (AED) and First Aid onsite during the entire event.
6. No items shall be attached to School District property nor any modifications made to the building structure,
equipment or grounds.
7. The organization will comply with fire codes, life safety codes and all other applicable local, state, and federal codes,
as well as all guidelines set forth in the Facility Rental Program Guide.
8. There will be no alcohol or narcotics on school property.
9. There will be no smoking, vaping, or any tobacco use allowed on school property.
10. No motorized vehicles are to be driven on school property. All vehicles must remain on paved parking lots, with the
exception of emergency vehicles.
11. I understand rentals will be canceled when emergency days are declared by School District U46.
12. I realize the Board of Education, or a representative thereof, can revoke the privilege of using the building should it
deem it necessary to do so for any reason.
13. 1 realize any school sponsored activity takes precedence over any outside scheduled activity.
14. 1 understand and agree that cancellations by the organization must be made 48 hours prior to the last business day of
the date the facility is to be used. Further, I understand and agree that for any cancellation by the organization that
is less than 48-hours' notice, I agree to forfeit the rental deposit.
15. I understand that custodians will start one (1) half hour before the scheduled rental and work as necessary after the
rental. An additional half hour will be applied before and after the rental event to cover this cost. And N the event is
involving food or hosting 150 or more participants requires two custodians present at the event.
16. 1 understand that at times and for certain rental activities School District U46 may require additional adult
supervision, chaperons, police, deposit for clean-up costs, etc. All additional expenses are the responsibility of the
renter.
17. No meetings shall be political in nature, be subversive to the U.S. Government, immoral, or discriminate against any
race, color, creed, or sex.
18. The Wsu4hution of any literature or materials must have prior approval of the Board of Education or designee.
