RESOLUTION
AUTHORIZING THE EXECUTION OF A GRANT AWARD AGREEMENT
FROM THE ILLINOIS EMERGENCY MANAGEMENT AGENCY FOR THE
2004 CITIZEN CORPS PROGRAM
BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF ELGIN,ILLINOIS,that
Ed Schock,Mayor,be and is hereby authorized and directed to execute a grant award agreement with
the Illinois Emergency Management Agency for the 2004 Citizen Corps Program,a copy of which is
attached hereto and made a part hereof by reference.

NOTICE OF GRANT AGREEMENT
Part I -Notice of Grant Award to City of Elgin
This Grant Agreement is made and entered by and between the Illinois Emergency Management
Agency (Grantor), 110 East Adams Street, Springfield, Illinois 62701-1109, and City of Elgin
(Grantee), 150 Dextor Court, Elgin, Illinois 60120-5555.
WHEREAS this Grant is to utilize funds from the Department of Homeland Security (DHS)
Office for Domestic Preparedness (ODP) Fiscal Year 2004 Appropriation for the Citizen Corps
Program.
THEREFORE, the Grantor is hereby making available to the Grantee the amount not exceeding
$21,487.00 for the period from the date of final execution to November 30, 2005. The Grantee
hereby agrees to use the funds provided under the agreement for the purposes set forth herein and
agrees to comply with all terms and conditions of this agreement. This period of award may be
amended if there is a delay in the release of these funds from the Federal Government.
It is agreed between the parties, that the agreement, as written, is the full and complete
agreement between the parties and that there are no oral agreements or understanding between
the parties other than what has been reduced to writing herein.
This Grant Agreement and Attachments constitutes the entire agreement between the parties.
Each budget detail worksheet submitted by the Grantee and approved by the Grantor as an
authorized expenditure of this Grant shall be considered an attachment of this Grant Agreement.
Part II - Term
The term of this Grant Agreement shall be from the date of final execution by the Grantor
through November 30, 2005.
Part III - Scope of Work
During the grant period, funds received by the Grantee will be utilized by the Grantee to
administer the Citizen Corps Program.
Grantee strategy and work plan, are provided in Application for Funding (Attachment A.). As
each budget detail worksheet is approved, it shall also be considered as an attachment of this

Part IV - Compensation Amount
• The total compensation and reimbursement payable by the Grantor to the Grantee shall not
exceed the sum of$21,487.00 total amount.
Part V-Terms and Conditions
FISCAL FUNDING: The Grantor's obligations hereunder shall cease immediately, without
penalty or further payment being required, in any year for which the General Assembly of the
State of Illinois fails to make an appropriation sufficient to pay such obligation or ODP fails to
provide the funds. The Grantor shall give Grantee's notice of such termination for funding as
soon as practicable after Grantor becomes aware of the failure of funding. Grantee's obligation
to perform shall cease upon notice by Agency of lack of appropriated funds.
METHOD OF COMPENSATION: The method of compensation shall be reimbursement in
accordance with the invoice voucher procedures of the Office of the State of Illinois
Comptroller. The Grantee agrees to maintain appropriate records of actual costs incurred and to
submit expenditure information to the Grantor. No costs eligible under this agreement shall be
incurred after November 30,2005:
ACCOUNTING REQUIREMENTS: The Grantee shall maintain effective control and
accountability over all funds, equipment, property, and other assets under the Grant Agreement
as required by the Grantor. The Grantee shall keep records sufficient to permit the tracing of
funds to ensure that expenditures are made in accordance with this Grant Agreement.
REPORTS: The Grantee shall submit programmatic reports as may be required by the Grantor.
AUDITS AND INSPECTIONS: The Grantee will, as often as deemed necessary by the Grantor,
ODP or any of their duly authorized representatives, permit the Grantor, ODP or any of their
duly authorized representatives to have full access to and the right to examine any pertinent
books, documents, papers and records of the Grantee involving transactions related to this grant
agreement for three years from the date of submission of the final expenditure report or until
related audit findings have been resolved, whichever is later. The Grantee certifies that all audits
submitted under the provisions of Office of Management and Budget.Circulars A-128 or A-133
have been approved by the Grantor. The Grantee acknowledges that these are federal pass-
through funds that must be accounted for in the jurisdiction's Single Audit under the Single
Audit Act of 1996,if required.
MODIFICATION AND AMENDMENT OF THE GRANT: This grant agreement is subject to
revision as follows:
A. Modifications may be required because of changes in State or Federal

military service. Such action shall include, but not be limited to, the following: employment,
upgrading, demotion, or transfer; recruitment or recruitment advertising; layoff or termination;
rates of pay or other forms of compensation; and selection for training; including apprenticeship.
The Grantee shall post in conspicuous places, available to employees and applicants for
employment, notices to be provided by the Government setting forth the provisions of this non-
discrimination clause.
SEVERABILITY CLAUSE: If any provision under the Grant Agreement or its application to
any person of circumstance is held invalid by any court of competent jurisdiction, this invalidity
does not affect any other provision or its application of the Grant Agreement which can be given
effect without the invalid provision or application.
DEBARMENT: The Grantee certifies neither it nor its principals is presently debarred,
suspended, proposed for debarment, declared ineligible, or voluntarily excluded from
participation in the agreement by any Federal Agency or department.
WORKER'S COMPENSATION INSURANCE, SOCIAL SECURITY, RETIREMENT AND
HEALTH INSURANCE BENEFITS, AND TAXES: The Grantee shall provide worker's
compensation insurance where the same is required, and shall accept full responsibility for the
payment of unemployment insurance, premiums for worker's compensation, social security and
retirement and health insurance benefits, as well as all income tax deductions and any other taxes
or payroll deductions required by law for employees of the Grantee who are performing services
specified by the grant agreement.
WAIVERS: No waiver of any condition of this grant agreement may be effective unless in
writing from the Director of the Grantor.
BOYCOTT: The Grantee certifies that neither it nor any substantially-owned affiliated company
is participating or shall participate in an international boycott in violation of the provisions of the
U.S. Export Administration Act of 1979 or the regulations of the U.S. Department of Commerce
promulgated under that Act.
Part VI-Assurances
The Grantee assures that no official or employee of the Grantee who is authorized in the
Grantee's official capacity to negotiate, make, accept, or approve, or to take part in such
decisions regarding a contract for acquisition/development of property in connection with this
agreement, shall have any financial or other personal interest in any such contract for the
acquisition/development.
The Grantee will initiate and complete the work within the applicable time frame after receipt of
approval of the awarding agency.

The Grantee will comply, as applicable, with provisions of the Hatch Act (5 U.S.C. §§1501-1508
and 7324-7328) which limit the political activities of employees whose principal employment
activities are funded in whole or in part with Federal funds.
The Grantee will cause to be performed the required financial and compliance audits in
accordance with the Single Audit Act Amendments of 1996 and OMB Circular No. A-133,
"Audits of States, Local Governments, and Non-Profit Organizations."
The Grantee will comply with all applicable requirements of all other Federal laws, executive
orders, regulations, and policies governing this program.
Part VII- Certification
The Grantee certifies that it has not been convicted of bribery or attempting to bribe an officer or
employee of the State of Illinois, nor has any official, agent, or employee of the Grantee
committed bribery or attempted bribery on behalf of the Grantee and pursuant to the direction or
authorization of a responsible official of the Grantee.
The Grantee hereby certifies that it has not been barred from bidding on, or receiving State or
local government contracts as a result of illegal bid rigging or bid rotating as defined in the
Criminal Code of 1961 (720 ILCS 5/33E-3 and 33E-4).
The Grantee certifies that it will comply with all applicable State and Federal laws and
regulations.
The Grantee certifies that it will return to the State all State or Federal grant funds that are not
expended or are accidentally over-advanced. The State may recapture those funds not expended
or accidentally over-advanced in accordance with State and Federal laws and regulations. The
Grantee further certifies that its failure to comply with any one of the terms of this Grant
Agreement shall be cause for the Grantor to seek recovery of all or part of the grant proceeds.
The Grantee certifies that it will establish safeguards to prohibit employees, contractors, and
subcontractors from using their positions for a purpose that constitutes or presents the
appearance of personal or organizational conflict of interest, or personal gain.

Part VIII -Drug Free Certification
This certification is required by the Drug Free Workplace Act (30 ILCS 580). The Drug Free
Workplace Act, effective January 1, 1992, requires that no Grantor or contractor shall receive a
grant or be considered for the purposes of being awarded a contract for the procurement of any
property or services from the State unless that Grantor or contractor has certified to the State that
the Grantor or contractor will provide a drug free workplace. False certification or violation of
the certification may result in sanctions including, but not limited to, suspension of contract or
grant payments, termination of the contractor or grant and debarment of contracting or grant
opportunities with the State for at least one (1)year but not more than five(5)years.
For the purpose of this certification, "Grantor" or "contractor" means a corporation, partnership,
or other entity with twenty-five (25) or more employees at the time of issuing the grant, or a
department, division, or other unit thereof, directly responsible for the specific performance
under a contract or grant of$5,000 or more from the State.
The contractor/Grantor certifies and agrees that it will provide a drug free workplace by:
(a) Publishing a statement:
(1) Notifying employees that the unlawful manufacture, distribution, dispensing,
possession or use of a controlled substance, including cannabis, is prohibited in
the Grantor's or contractor's workplace.
(2) Specifying the actions that will be taken against employees for violations of such
prohibition.
(3) Notifying the employee that, as a condition of employment on such contract or
grant,the employee will: •
(A) abide by the terms of the statement; and
(B) Notify the employer of any criminal drug statute conviction for a violation
occurring in the workplace no later than five (5) days after such
conviction.
(b) Establishing a drug free awareness program to inform employees about:

(1) the dangers of drug abuse in the workplace;
(2) the Grantor's or contractor's policy of maintaining a drug free workplace;
(3) any available drug counseling, rehabilitation, and employee assistance programs;
and
(4) the penalties that may be imposed upon an employee for drug violations
(c) Providing a copy of the statement required by subparagraph (a) to each employee
engaged in the contract or grant and to post the statement in a prominent place in the
workplace.
(d) Notifying the contracting or granting Agency within ten (10) days after receiving notice
under part (B) of paragraph (3) of subsection (a) above from an employee or otherwise
receiving actual notice of such conviction.
(e) Imposing a sanction on, or requiring the satisfactory participation in a drug abuse
assistance or rehabilitation program by, any employee who is so convicted, as required by
section 5 of the Drug Free Workplace Act.
(f) Assisting employees in selecting a course of action in the event drug counseling,
treatment, and rehabilitation is required and indicating that a trained referral team is in
place.
(g) Making a good faith effort to continue to maintain a drug free workplace through
implementation of the Drug Free Workplace Act.

Grant Request Narrative
"To capture the spirit of service that has emerged throughout our communities, we launched the
USA Freedom Corps initiative earlier this year. As pad of that initiative, we created Citizen
Corps to help coordinate volunteer activities that will make our communities safer, stronger and
better prepared to respond to any emergency situation."

The Elgin Citizen Corps Council
The Elgin Citizens Corps Council (ECCC) is being developed to harness the power of the
citizens of Elgin, Illinois through education, training and volunteer service to make our
community safer, stronger, and better prepared to respond to the threats of terrorism, crime,
public health issues, and disasters, man-made or natural, of any kind.
The ECCC will be organized through a core of community based programs already in place and
providing community services to the emergency first responder organizations. The parent
organizations, of these groups, serve as the national chartered members of the Citizens Corps,
they are: Neighborhood Watch (Rope, Nope, ECN), VIPS (Citizens Police Academy Alumni
association and Law Enforcement Explorer Post 1445, Medical Reserve Corps (Sherman
Hospital, St. Joseph Hospital, Kane County Public Health) and our newest member, CERT
(Community Emergency Response Teams).
These four community groups will continue to develop, build and promote community-based
programs that build on our strengths and implement plans for our citizens to participate through:
Personal Responsibility: Developing household preparedness plans and disaster supply kits,
observing home health and safety practices, implementing disaster mitigation measures, and
participating in crime prevention and reporting.
Training: Taking classes in emergency preparedness, response capabilities, first aid, CPR/AED,
fire suppression, and search and rescue procedures.
Volunteer Services: Engaging in volunteer activities that support our first responders, disaster
relief groups, and community safety organizations.
Together they will form the Elgin Citizen Corps Council to support Elgin's law enforcement,fire,
emergency medical services, community public health efforts, and the four stages of emergency
management: prevention, mitigation, response, and recovery
Program and Financial Need
The City of Elgin is the largest of the Northwest suburbs of Chicago. The City is in the midst of a
great transition, perhaps the greatest in its modem history. After many decades of modest
change, Elgin's growth is beginning to accelerate sharply. Elgin's population forecasts for the
next ten years suggest a projected population of more than 145,000. This will be an increase of
more than 50% from the current population of 95,000. This growth is welcomed by some as an
opportunity for progress, and yet it is also feared by others as a major change.
Concerns over taxes and financial stability accompany this change. Due to the worst economic
times in the last two decades the residents and city government have been forced to adopt a "do
more with less" environment in regards to budgets, expenditures and city services. The need for
community involvement has never been in higher demand.
Following the events of 9/11, Elgin has been even more committed to using its citizen resources
to interface with the City's emergency first responders. Community programs involving Citizen
Corp national partners such as Neighborhood Watch, Volunteers in Policing Services(VIPS)
with the Citizens Police Academy and the Law Enforcement Explorer Post#1445, Medical
Reserve Corp, and Community Emergency Response Teams (CERT) have volunteered
thousands of community service hours annually to support Elgin's emergency first responder
programs.
It is fully recognized that, despite the City's high level of professional emergency services and
preparedness training, it is realized that the first responding emergency personnel could quickly
become inundated in the event of a major disaster, even with its extensive mutual aid program in
place. The concept of"Neighbors Helping Neighbors: become most important during these
times. Having disaster trained and certified citizens to help each other stabilize their
neighborhoods will allow the emergency first responders to focus on the most critical of
situations facing the City following a disaster of any type.
Until the availability of financial assistance from the Illinois Terrorism Task Force and the Illinois
Emergency Management Agency no funds were available for this endeavor. Elgin is proud to
have been recently recognized as one of seven Illinois communities to receive a 2003 grant for
developing,training, and equipping a Community Emergency Response Team. Funding
received from the Citizen Corp Council grant will provide the additional funds required to
complete the CERT training and equipment program as well as provide for establishing
community outreach and public education programs in conjunction with the core partners of
Citizen Corps.
Elgin Citizen Corps Manaqement
One of the first organizational steps of the ECCC will be to determine a mission statement for
the group and hold a strategic goals and planning workshop for it's members. Through these
efforts the ECCC will develop it's short and long-range planning efforts in the areas of
Management. Community Outreach, Funding and Programs. Expectations for these areas
include:
Management: The affairs of the ECCC will be vested in its Board of Directors who are non-
compensated volunteers dedicated to promoting the mission and goals of the ECCC. The Board
will be comprised of eleven voting members who collectively represent the ECCC programs
(NW, CERT, MRC, and VIPS), the City of Elgin, and community, business, and volunteer
leaders from the Elgin area. Ex-officio members of the Board will complete the overall vision of a
collaborative body of experience and leadership of like-minded leaders of Elgin.
Community Outreach: The outreach committee of the ECCC will be charged with raising the
public awareness of ECCC by distributing informational and educational materials, attending
community events, and speaking at key opportunities to promote the ECCC mission and
fundraising efforts to Elgin's residents.
The outreach Committee will also work to collaborate the efforts of the ECCC with partners and
representatives from various city departments, allied public, private, and volunteer agencies
including the American Red Cross, United Way, Salvation Army, Senior Triad, Boy Scouts of
America, businesses, faith-based groups and residents to develop a community wide plan for
educating all residents of Elgin in a "Neighbors Helping Neighbors" program to ask, "ARE YOU

READY ELGIN?". The primary goal of which will be to have all of our residents- "Make a Kit",
"Make A Plan", and "Be Informed".
Funding: The Budget and Finance Committee will be responsible for annual budget preparation
and expenditure review, financial accounting and auditing, fundraising initiatives and fiscal policy
implementation for the ECCC and its programs. The Budget and Finance committee will provide
oversight for the distribution of$20,000 in funds from the 2003 Community Emergency
Response Team (CERT) grant that was recently awarded to the City of Elgin from the Illinois
Emergency Management Agency. In addition outside funding will be sought to fund initiatives
through solicitations for local private donations, and corporate community partnerships with
Home Depot (Home Depot foundation) and McDonalds (McReady for Disaster Program).
Programs: The Programs Committee will affirm the training efforts for the CERT program now
funded and beginning instructor(Train-the-Trainer) education. The Committee will also work
with the four core ECCC programs and other citizens to encourage participation in volunteer
programs. The Programs Committee will develop a community event and CERT team exercise
for the Neighborhood Watch "National Night Out"program focusing on the theme of"Are You
Ready- Elgin?" and the concepts of-"Making a Kit, Making a Plan, and Being Informed"
Elqin's Citizen Corps Council Development Phases
Phase 1 ❑ Initial representative sent to IEMA to attend the CERT Train-the-Trainer
course.
❑ Establish understanding with City Officials to proceed with Community CERT
Training.
❑ Preview core partners (Neighborhood Watch, VIPS, MRC) on the ECCC
program.
❑ Apply for CERT and Citizen Corps Council grants from IEMA.
❑ Formally introduce Citizen Corps and CERT to the Elgin Community on
February 28, 2004, during the City's 150th Sesquicentennial Anniversary Open
House.
• This phase will be completed on February 28, 2004 with the introduction of ECCC and
CERT to the Elgin Community. Steve Figved, a volunteer in the Police Department's CR/CP
Division, has agreed to serve as Project Coordinator and point-of-contact for the program.
The City of Elgin is extremely proud to have been selected as one of seven communities to
receive a 2003 Illinois Emergency Management Agency (IEMA) CERT Grant for$20,000.
Phase 2 ❑ Send two additional instructors (one police/one fire)to IEMA Train-the-Trainer
certification in April of 2004.
❑ Begin organization of the Elgin Citizens Corp Council from contacts received
from city officials, partner members, and other community representatives.
❑ Produce course materials including instructor and student manuals along
with miscellaneous handouts.
Produce outreach promotional and educational materials for use with core
partner groups.
❑ Obtain equipment and supplies for classroom, audio-visual, fire suppression,
medical and search/rescue materials to be used in presenting training and the
coordination of the program.
❑ Develop member tracking and equipment inventory database on ECCC
computer.
❑ Promote ECCC programs through City Web-site, local press and radio
contacts.

Phase 3 0 Complete a "pilot"CERT class for core partner members by June 15, 2004.
❑ Train and additional twenty-five (25) CERT members from the core partners
by August 15, 2004.
O Obtain CERT Personal Response Pack items to establish fifty (50) response
packs.
❑ Maintain documentation of budget and expenditures under the CERT grant.
Respond to Grant program requirements including submittal of Quarterly
Program Activity Reports to IEMA.
❑ Prepare and distribute a Community Risk and Resource Assessment to help
identify high-risk areas of the community and potential multi-hazard threats. This
assessment will be consolidated with other emergency management projects
already in place within the core partner groups, the City of Elgin, emergency first
responders and other disaster oriented agencies such as the American Red
Cross.
❑ Coordinate the "Neighbors Helping Neighbors" project-Are You Ready Elgin?,
using the Community Risk Assessment data to be introduced in a "Kick-Off
Program"at the Neighborhood Watch "National Night Out" in August 2004.
Phase 4 ❑Obtain funding for maintaining and expanding CERT program to include
- a third CERT training Program for twenty-five additional core partner
members.
- purchase of equipment and supplies to bring response pack and
personal safety equipment inventory to seventy-five (75) units.
❑ Provide continued support to community outreach "Neighbors Helping
Neighbors" program for-"Are You Ready Elgin?"
❑ Provide continued support to core partners and other community groups to
promote the ECCC's initiatives.
❑ Plan for a CERT community exercise to be held during National Night Out
activities in August 2005.
Phase 5 0 Expand Outreach program "Neighbors helping neighbors"-Are You Ready
Elgin?for providing public education to business, school-based, high-risk,
special needs, and the Hispanic,African-American, and Laotian groups and
neighborhoods.
O Maintain funding sources and local, corporate sponsorships for ECCC
programs and initiatives.
O Maintain regional, state and national contacts and relationships with Citizen
Corp Council core partner groups and funding agencies.
❑ Work to obtain and maintain participation from local first responders to
identify their emergency response needs and provide highly trained and reliable
volunteers to assist and integrate with the first responders in emergency
situations.
❑ Focus on maintaining ECCC and CERt membership by designing team
exercises and offering advanced, technical training.
❑ Continue public education efforts through coordinating neighborhood events
and community-based activities targeted at emergency disaster response and
"Neighbors Helping Neighbors", Are You Ready-Elgin?", ECCC newsletters,
publications and other information and tools.
Use of Grant Funds
IEMA grant funds will be used exclusively for the development of our Citizen Corps Council, the
continued delivery of the CERT training program, and establishing and maintaining community
outreach and education programs on disaster preparedness.

The grant funds will be used to form the Citizen Corp Council and manage its organizational
activities and initiatives. Funds for a volunteer project coordinator will be used to assist the
ECCC in determining its mission statement and holding a strategic planning workshop. The
project coordinator will also serve as a liaison to the core partner members and provide
materials and community education and information to the various partners.
The funds designated for CERT training will include the necessary materials and equipment to
present the training, as well as compensation for instructor preparation and delivery time. A third
CERT program will be presented to the core partner members. Upon their completion of the
course, Elgin will have certified, trained, and equipped seventy-five (75) CERT members. The
training funds covered by the grant will also provide for a CERT training exercise to be included
in the Neighborhood Watch "National Night Out" program in August of 2005.
Additional funds will be set aside for community outreach and public education programs based
upon Elgin's "Neighbors Helping Neighbors"theme and the "Are You Ready Elgin?" project.
These projects are designed to teach our citizens that disaster can strike when you least expect
it. "Are You Ready Elgin?" will provide public education and reduced cost supplies to"Make a
Kit" of emergency supplies including; water, food,first aid supplies and special need items.
Participants will also leam how to "Make a Plan"for what they will do in an emergency situation
including; creating a family plan, deciding whether to stay or go, what to do at work, school or
home, and what to do in a moving vehicle. Finally, those involved will learn to "Be Informed"
about what to do before, during, and after a disaster strikes as well as what type of disaster
situations could occur in Elgin. Information gathered from Community Risk Assessments and
Disaster Questionnaires will be used a resource in developing the programs.
Proqram Expenditures
A detailed budget is provided as part of this Grant Request Narrative. The Elgin Citizens Corp
Council would maintain documentation of a budget and expenditures under this grant. The
budget would be established to cover a two-year period of March 1, 2004 through December 31,
2005 as such;
2004 Citizen Corps Budget and Expenditures- $ 9965 (46% of funding)
