RESOLUTION
ACCEPTING THE PROPOSAL OF TRUST MARK INSURANCE COMPANY
FOR STOP LOSS AND AGGREGATE INSURANCE COVERAGE IN CONNECTION
WITH THE CITY OF ELGIN' S MEDICAL INSURANCE PPO PLAN
AND AUTHORIZING THE EXECUTION OF DOCUMENTS RELATED THERETO
BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF ELGIN,
ILLINOIS, that the City of Elgin hereby accepts the proposal of
Trust Mark Insurance Company for stop loss and aggregate insurance
coverage in connection with the City of Elgin' s medical insurance
PPO plan for the period of March 1, 2003 through February 29, 2004 .
BE IT FURTHER RESOLVED that Olufemi Folarin, Interim City
Manager, be and is hereby authorized and directed to execute all
documents necessary and incident to such proposal of Trust Mark
Insurance Company.
