RESOLUTION
APPROVING CHANGE ORDER NO. 1 IN THE CONTRACT WITH
MARTAM CONSTRUCTION, INC. FOR SIDEWALK REPAIRS AT CITY HALL
WHEREAS, the City Elgin El in has heretofore entered into a
contract with Martam Construction, Inc . for sidewalk repairs at
city hall ; and
WHEREAS, it is necessary and desirable to modify the terms of
the contract as is described in Change Order No. 1 , attached
hereto.
NOW, THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY
OF ELGIN ILLINOIS, that Joyce A. Parker, City Manager, be and is
hereby authorized and directed to execute Change Order No. 1, a
copy of which is attached hereto and made a part hereof by
reference .

CHANGE ORDER NO. 1 and Final
This Change Order made and entered into this 25th day of April, 2001, by and between the City of
Elgin, an Illinois municipal corporation, and Madam Construction, Inc.
ORIGINAL CONTRACT: Sidewalk Repairs at City Hall
Invitation to Bid No. 00-080A
Contract Date September 19, 2000
PROJECT NO. 379513
SCOPE: Replace damaged drains, repair damaged stairs, and pour additional section
of concrete sidewalk.
REASONS FOR CHANGE: After removal of the old concrete and gravel it was
discovered that the drains were in very poor condition and needed to be replaced. The
stairs to the plaza from the west entrance of City Hall deteriorated with the freeze thaw
cycles over the winter and have become a severe safety hazard. This work would be
for repairs only not replacement. The additional sidewalk work is to provide a slower
elevation change needed due to the change in scope from the original design.
SUMMARY OF CHANGES IN CONTRACT AMOUNT: The amount of the contract will
be revised as follows:

Furnish and install 8 drains @ $800.00 each $ 6,400.00
Repair concrete stairs $ 850.00
Replace 300 sq. ft. of concrete sidewalk $ 1,800.00
TOTAL AMOUNT OF CHANGE ORDER # 1 $ 9050.00
Original Contract Price Including Alternates $ 116,040.00
New Contract Amount $ 125,090.00
OTHER CONTRACT PROVISIONS: All other contract provisions shall remain the
same.

SUBJECT: Change Order No. 1 - City Hall
Sidewalk Replacement
PURPOSE
The purpose of this memorandum is to provide the Mayor and the
members of the City Council with information to approve a change
order to the City Hall Sidewalk Replacement Project .
BACKGROUND
The sidewalk at the main entrance to City Hall was badly
deteriorating and scheduled for replacement . A bid was awarded to
Martam Construction of Elgin at the September 13 , 2000 City Council
meeting. After removal of the existing concrete and gravel it was
discovered that the drains were in very poor condition and would
need to be replaced. Also due to a reduction in the scope from the
original design it was determined that the transition from the new
sidewalk to the old sidewalk would be too steep of a slope and an
additional section of concrete would need to be replaced to correct
this problem. The stairs to the west of the building that lead down
to the plaza have deteriorated severely during the freeze/thaw
cycles of the last winter and have become a severe trip hazard. The
work on the stairs would be a repair only, not a replacement . A
copy of the change order is attached for your review.

FINANCIAL IMPACT
The total budget for this project equals $150, 000 . Martam
Construction' s original contract totals $116, 040 . Change order No.
1 calls for an additional $9, 050, bringing the revised contract
total to $125, 090 . Additional miscellaneous expenses of $1, 810
bring the total project cost to $126, 900 .
