FOR THE NORTH MCLEAN BOULEVARD RECONSTRUCTION PROJECT WHEREAS, the City of Elgin has heretofore entered into a contract with Plote Companies, Inc . for the North McLean Boulevard reconstruction project ; and WHEREAS, it is necessary and desirable to modify the terms of the contract as is described in Change Order No. 1, attached hereto. NOW, THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF ELGIN, ILLINOIS, that Joyce A. The general contractor for this project is Plote Companies, Inc. of Elgin, IL. REASONS FOR CHANGE: The circumstances which necessitate the change in performance were not reasonably foreseeable at the time the contract was signed. SUMMARY OF CHANGES IN CONTRACT AMOUNT: The amount the contract will be revised is as follows: See Attachment A The original contract with Plote Companies, Inc. is $1,976,315.71. By reason of Change Order No. 1, the contract for the North McLean Blvd. Reconstruction Project will decrease in the amount of two thousand six hundred eighty-two and 48/100 dollars ($2,682.48). Project Engineer: Steve Pertzborn BC 22(REV 3-89) IL 494-0390 (20M-3/89) Mclean Blvd., Wing St. to Royal Ave. City of Elgin District 1 Cook County Change Order No. 1 New Items-Explanation of Reason for Change: Item No. X0000101 Type C Catch Basin, TY 10 F&G* The plans originally called for all four foot diameter catch basin (Type A) structures to be constructed. In some locations, due to conflicts with the location of existing utilities and the inability of the utility to be relocated in a timely fashion, it was necessary to utilize a smaller two foot diameter catch basin (Type C)to complete the work. ($5,820.00 increase) Item No. X0000102 Traffic Control Revision* To more safely move the motoring traffic through Stage 4 and to create a larger work zone for the contractor, Stage 4 was revised too provide a work zone shoulder on both sides of the road. To accomplish this additional barricades, drums and reflector were required. This item represent the additional cost for providing the revised staging. (S4,400.00 increase) Item No. X0000103 Remove Monolithic Curb* It was anticipated that the sidewalk and curb and gutter to be removed over the top of the culvert at approximately Sta. 70+75 was poured separately. When the contractor began to remove the sidewalk and curb and gutter, it was discovered that the sidewalk and curb and gutter were poured monolithic with portions of the culvert. The sidewalk and curb and gutter had to be carefully removed so as not to damage any portion of the concrete culvert. ($4,667.78 increase) Item No. X0000104 Gate Valve - 8"* The City of Elgin requested that an additional valve be installed at Sta. 76+80 for a potential development.($1,210.00 increase) Item No. X0000105 Down Time Due to Water Main Conflict* While attempting to construct the new water main and casing pipe under the existing culvert at approximately Sta. 70+75, an old casing pipe was discovered that was in the way of the new one. The location for the new main was adjusted to try to avoid the old casing, but a small portion of the casing had to be removed. To keep the phone duct from collapsing the contractor had to shore the duct. It was also discovered that the alignment of the existing water main was not where it was anticipated on the plans. The contractor had to do additional shoring to keep the existing water main from separating .The contractor had to bring in a contractor specializing in boring and auguring operation to finish the work. (54,393.96 increase) Item No. X0000106 Underdrain - 4"* Due to the lack off cover, the culvert manufacturer recommended that the 12" aggregate base course not be constructed over the top of the existing culvert. To minimize the potential for a drainage problem at the existing culvert as a result of the base course subgrade elevation differential, a four inch underdrain was constructed on the down hill grade transverse to the existing culvert. ($1,062.00 increase) Item No. X0000107 Water Service Connection* The plans originally called for the new water service lines to be connected to the existing water serve valves and boxes. The City Water Department decided that the existing valves were old and in poor condition and requested that all new water service valves and boxes be installed. ($9,360.00 increase) Item No. X0000108 Bushes* • Because of limited space, to construct the sidewalk adjacent to the strip mall on the southeast corner of Wing Street and McLean Blvd., the existing bushes and landscaping needed to be removed. This item reflects the replacement of the existing bushes. (5787.50 increase) Item No. X0000109 Area Reflective Crack Control* Strip Reflective Crack Control consisting of an eighteen inch wide geotechnical fabric was to be placed on the transverse and longitudinal cracks on thru the grinding and overlay section of McLean Blvd. Although the pavement was in fair condition and required no patching, the cracks were numerous. It was decided in lieu placing a "strip" fabric on the individual cracks to use an "area" Reflective Crack Control treatment to cover the entire width of pavement. (S7,521.15 G:/265/constr/changeorder/mcleanco 1.wpd 2 Mclean Blvd.,Wing St. to Royal Ave. City of Elgin District 1 Cook County Change Order No. 1 New Items-Explanation of Reason for Change: increase) Item No. X0000110 Sanitary Manhole Removal and Pipe Abandonment* The plans originally called for two sanitary manholes at Sta. 10+00 Wing Street to be adjusted. The manhole contained valves that were part of an old pressure system. During construction there was confusion if these were actually water valves (as marked by the City Water Department) or sanitary valves as indicated on our plans. The contractor was requested to perform exploratory excavation to determine the exact location of the watermain in this area. Once the watermains were located the contractor was requested to "tap" the existing sewer pipe to determine if it was live or abandoned. The Sanitary Department determined that it was abandoned and requested that the sanitary manholes and valves be removed and the pipe be capped with mechanical joint plugs. The resultant excavation required backfilling with granular embankment since it was all with the pavement limits. ($4,689.90 increase) Item No. X0000111 Reconstruct Drainage Structure* The contractor installed Storm Drainage Structure D-10 at the location shown on the plan. However, do to an existing utility conflict with the phone duct, minor adjustments were required to the taper that the structure was original constructed in. The contractor was requested to remove and relocate the drainage structure to a revised location. ($1,152.75 increase) Item No. X0000112 Chain Link Fence, Sidewalk Mounted - 4'* There were concerns regarding the narrow sidewalk that was adjacent to the bank parking lot and the street at the southwest corner Wing Street and McLean Blvd.. Aligning and providing a natural barrier between the sidewalk and the bank parking lot was a hedge of bushes. To allow for the construction of a wider sidewalk at this location, the bank allowed for the removal of the bushes but required a four foot chain link fence to be constructed adjacent to the parking lot. The fence had to be anchored to the sidewalk. (S1,612.00 increase) Item No. X0000113 Chain Link Fence Gate, 4'x10'* An existing chain link fence was removed and replaced on the east side of McLean Blvd. from G:/265/constr/changeorder/mcleanco l.wpd 3 Mclean Blvd., Wing St. to Royal Ave. City of Elgin District 1 Cook County Change Order No. 1 New Items-Explanation of Reason for Change: Sta. 79979 to Sta. ????. There was an existing 4' x 10' wide chain link fence gate also removed as part of this work. A new gate had to be install to provide access to private property on the north and south side of the culvert. There was no provision in the plans for this gate. ($720.00 increase) Item No. X0000114 Chain Link Fence Gate, 4'x12'' See explanation for Item X0000114. ($750.00 increase) Item No. X0000115 Reflective Sign Material * The City of Elgin requested that the "School" warning signs be made with diamond grade sheeting in lieu of the standard engineering grade reflective material. This item reflects the additional cost of this reflective media. ($295.31 increase) Item No. X0000116 Water Main Repair* A water main occurred within the project limits but was unrelated to construction work. The City of Elgin requested that the contractor perform the repair since he was already mobilized and on- site. Parker, City Manager SUBJECT: Authorization of Change Order No. 1 North McLean Boulevard Reconstruction Project PURPOSE The purpose of this memorandum is to provide the Mayor and members of the City Council with information to consider authorization of Change Order No. 1 for the subject project . BACKGROUND At its February 10, 1999 meeting, City Council approved the contract with Plote Companies for the reconstruction of N. McLean Boulevard from Kaskaskia Avenue to Abbott Drive. The project completely reconstructed North McLean Boulevard from Kaskaskia Avenue to Royal Boulevard. It includes a new roadway base and surface, new storm sewer system, rehabilitation of the bridge over Tyler Creek, new traffic signals at the North McLean Boulevard/Royal Avenue and North McLean Boulevard/Wing Street intersections and a pedestrian handrail for sections of the sidewalk that could not be separated from the curb line . In addition North McLean Boulevard from Royal Boulevard to Abbott Drive was ground and resurfaced with 1 . 5" of new asphalt . Change Order No. 1 will add $49, 703 . 12 in new items to the contract . These new items were not reasonably foreseen at the time the contract was bid and are necessary to complete the project in a reasonable manner. This change order also includes the deletion of $52 , 385 . 60 in existing items from the contract . Thus, Change Order No. 1 will result in a net decrease to the contract in the amount of $2, 682 .48 . For an explanation of each change order item, please see attachment A of Exhibit B at the end of this memo. The current contract with Plote Companies is in the amount of $1, 976, 315 . 71 . The new contract amount will be $1, 973 , 633 . 23 . Payments made to Plote Companies to date total $1, 710, 698 .41, leaving a balance of $262, 934 . 82 . eft- A letter of recommendation of approval of Change Order No. 1 from Crawford, Murphy and Tilly (CMT) , the engineering firm hired by the City to provide engineering services, is attached as Exhibit A.
