Kaptain David J. Kaptain, Mayor Presented: January 14, 2015 Adopted: January 14, 2015 Omnibus Vote: Yeas: 9 Nays: 0 Attest: s/Kimberly Dewis Kimberly Dewis, City Clerk Change Order V City of Elgin ELGIN,ILLINOIS PROJECT FIRE DEPARTMENT PROTECTIVE TURNOUT CLOTHING CHANGE ORDER NO. 1 AND FINAL CONTRACT: Fire department protective turnout clothing is the clothing that is worn by firefighters during fires. National Fire Protection Association (NFPA) standard 1851 requires turnout clothing to be replaced every ten years to maintain its effectiveness and to ensure it meets current minimum standards. RFP Number 13-070 SCOPE: This change order allows for the schedule replacement purchase of the protective turnout clothing. REASONS FOR CHANGE: The cause of this change order request was the retirement of seven fire department personnel in 2014. This required the hiring of seven new firefighters, which also required the purchase of 14 sets of protective turn-out gear. One set of protective gear is utilized for the fire academy, and the other set is utilized when the firefighters begin working on their shift. The circum- stances said to necessitate the change in performance were not reasonably foreseeable at the time the contract was signed, and; the change is germane to the original contract as signed, and; the change order is in the best interests of the City of Elgin and authorized by law. SUMMARY OF CHANGES IN CONTRACT AMOUNT: The contract is being amended to provide the additional sets of protective turnout clothing. ORIGINAL CONTRACT AMOUNT: $83,350 CHANGE ORDER NO. 1 Purchase of sixteen sets of protective turnout clothing $17,900 TOTAL FOR CHANGE ORDER NO. 1: ADD $17,900 TOTAL REVISED CONTRACT AMOUNT: $101,250 The original amount of the contract with Equipment Management Company is $83,350. Change Order No. 1 will increase the original contract amount by a total of$17,900, that sum representing twenty-one and forty-eight hundredths percent (21.48%) of the original contract amount. The new contract amount, inclusive of all change orders is $101,250. OTHER CONTRACT PROVISIONS: All other contract provisions remain the same.
