RE: Public Health Division 2012 Year End Report
The Public Health Division of the Community Development Group currently licenses and
inspects 402 food handling establishments in Elgin, more than twice as many establishments as
twenty years ago. The increase of over 130 food handling establishments since 2000 is attributed
to retail development in Elgin, as well as the licensing of soup kettle kitchens, school cafeterias,
and daycare centers.
A food handling license is required for all food service establishments and retail food stores.
This includes full service restaurants, fast food restaurants, catering operations, school cafeterias,
nursing home kitchen facilities, daycare centers, workplace cafeterias, grocery stores, bakeries,
meat markets, taverns and convenience stores. Each establishment is assigned a category of
high,moderate or low relative risk of causing a food borne illness, and inspection frequencies are
assigned accordingly.
High risk establishments are inspected at least two or three times per year. In general, full
service restaurants, nursing homes, and catering operations are classified as relatively high risk
because their food is usually either cooked, cooled and reheated for later use; prepared for off-
site service (e.g. catering); prepared hot or cold and held more than 12 hours in advance of
service; prepared requiring complex or extensive handling; or served to immunocompromised
individuals.
Moderate risk establishments are normally inspected annually. Fast food facilities, grocery
stores, bakeries, meat markets, and convenience stores are generally designated as moderate risk
since they serve food that is held for short periods of time, or with no cooling and reheating.
Low risk establishments have a relatively low risk of causing food-borne illness and are
inspected annually. Taverns, liquor stores, and retail food stores selling only prepackaged foods
are examples of low risk establishments.

The Public Health Division issued a total of 98 temporary and seasonal food licenses the past
year. Temporary food licenses are food service establishments that operate at a fixed location for
less than 14 consecutive days in conjunction with a single event or celebration. Seasonal food
licenses are issued to businesses with multiple temporary events within the City. The increase in
temporary food licenses is due to the growth in the number of festivals, farmers markets, and
other special events that sell food. Each establishment is inspected and approved before it is
allowed to sell food to the public.

The Public Health Division inspected 50 scavenger trucks in 2012. The number of annual issued
licenses issued has steadily increased with the number of businesses requiring waste removal in
Elgin. Scavenger trucks are vehicles that haul, collect, or dispose of garbage, trash, ashes,
animal, human or vegetable refuse, or offal within the City limits. Trucks are inspected annually
for proof of title, proof of liability and property damage insurance, a valid license plate, a valid
state or federal safety number, and safety and sanitation concerns.
The Public Health Division also conducted 141 inspections of public swimming facilities in
2012. Public swimming pools are pools within the City open to the public and include
swimming pools operated by the City of Elgin, hotels, schools, apartment complexes, and
townhouse or homeowner associations. Swimming pools are inspected at least biannually, and
water samples are taken annually at each pool and tested for coliform bacteria. The number of
licensed swimming facilities has increased by almost 30% since 2000. Since 2003, the Health
Division has also inspected public spas to ensure they met Illinois Department of Public Health
requirements, which has increased the time spent at several swimming facilities.
The Public Health Division conducted 28 daycare center and 5 group home inspections in 2012.
Daycare centers are facilities that provide childcare for less than 24 hours per day for more than
3 children in a facility that is not a home. Group homes are child care facilities which provide
care for up to 10 children placed by and under the supervision of a licensed child welfare agency.
These homes are owned, staffed, maintained and operated by the Illinois Department of Children
and Family Services. Daycare centers are inspected to ensure compliance with the Illinois DCFS
licensing standards. Group Homes are inspected with the assistance of a Code Enforcement
Officer for property maintenance violations.
In addition, the Public Health Division inspected 52 vending machine locations in 2012.
Vending machines are inspected annually to ensure cold food machines are holding potentially
hazardous food at 41°F or less and cupped beverage machines are sanitary.
The Public Health Division worked in conjunction with Clarke to provide environmentally sound
mosquito larvae control in the Kane County portion of Elgin through complete inspections of
potential mosquito breeding sites, pre-hatch applications of active sites, larval control of
mosquitoes with insect growth regulators, and larval control treatment of the 5,500 public catch
basins. The Health Division conducted 29 inspections of stagnant water complaints in 2012 and
is licensed by the Illinois Department of Agriculture to apply mosquito larvacide on private
property when necessary.

• Thirty five food handling establishments opened and twenty closed. One daycare center
opened. Twenty one food handling establishment plans were reviewed.
• The Public Health Division investigated 86 food handling establishment complaints, 22
of which were suspected food-borne illness complaints.
• Thirty six other complaints were investigated regarding various health code violations
such as air, water, light and noise pollution, insect or rodent infestation, and stagnant
residential swimming pools.
• The Public Health Division temporarily suspended 2 food service establishment licenses.
In addition, approximately 335 pounds of food were discarded from food service
establishments on 20 separate occasions due to temperature violations or spoilage. In
addition, approximately$35,000 of food was discarded at Meijer due to a loss of power.
• The Public Health Coordinator conducted a food worker training to help establishments
comply with the new food service worker requirement. Since in 2011, the Elgin Food
Safety Code requires that all food service workers must complete a food service worker's
sanitation course and pass a test within 90 days of employment.
• The Public Health Coordinator attended 9 adjudication hearings for 2 separate cases. The
owner was found liable in each instance and fines totaling$3,500.00 were assessed.

The Elgin Board of Health quarterly meeting was held on Monday, January 28, 2013 in
the Community Development Conference room at City Hall. Edward Whitcomb
nominated Dr. Dorothea Poulos to act as chair pro tern. The motion passed four yes and
zero no. The meeting was called to order by Dr. Poulos at 7:34 p.m.

Election of a Chairperson: Edward Whitcomb made a motion to nominate Dr.
Dorothea Poulos as chairperson of the Board of Health. Cynthia Applehoff-Siedsma
seconded the motion. The motion was adopted by a vote of four yes and zero no.
Approval of Minutes: Cynthia Applehoff-Siedsma moved to approve the minutes from
the October 8, 2012 Board of Health quarterly meeting as submitted. Dr. Franz
Stockman seconded the motion. The motion was adopted by a vote of three yes, zero no,
and one abstention.

REVIEW OF THE FOURTH QUARTER OF 2012
Elgin Shell at the Grove, Pita Puff, Chooch's Pizza, M Squared Bakery, On the Side,

handling establishments. Danny's Pizza moved from South McLean Boulevard to
Douglas Street, and Chicago Pizza Authority moved from 840B Summit Street to 1050 A
Summit Street. Maedas Firepit closed and reopened as Elgin BBQ Pit. Within the Gail
Borden Public Library, Chicago Gourmet Café closed and was reopened as In the
Neighborhood Deli. Also, the Salvation Army moved its food packaging facility to the
Hemmens kitchen.
During the last three months of 2012 Elgin House of Pancakes, Douglas Street Bar and
Grill, and Azteca De Oro closed. Eight pounds of food was discarded from Bennigans
and 6 pounds of food was discarded from Elgin Community Resource Center due to
temperature violations. The Public Health Coordinator attended several adjudication
hearings for La Quebrada for a dumpster leaking onto a public sidewalk, and for Groot
Recycling and Waste Services for a scavenger truck leaking effluent onto private
property. Fines totaling over$3,500 were assessed in these cases.

REVIEW OF THE YEAREND REPORT
Mr. Bohner discussed the Public Health Division yearend report with the Board of
Health, and he noted that the number of licensed food handling establishments has
doubled in the past twenty years to over 400. Thirty six food handling establishments
opened in 2012 and twenty closed. In addition, the number of temporary and seasonal
temporary licenses issued annually has also doubled in the past eight years. Mr. Bohner
also discussed the inspections the Health Division conducts each year for scavenger
trucks, public swimming facilities, daycare centers, group homes, and food vending
machines.
MISCELLANEOUS
The mosquito control contract was awarded to Vector Disease Control International
(VDCI) by the City Council last week. Clarke had provided mosquito control for the
City since at least 1982, but VDCI proposed to provide similar control efforts at a much
lower price.
The City Council will most likely adopt a code of conduct for members of all city boards,
committees and commissions to provide guidance to members with their conduct while
serving the City of Elgin. This code of conduct may be in effect by the next Board of
Health meeting.
Mr. Bohner reminded the Dr. Stockman that he must submit his certificate for the Open
Meetings Act as soon as possible. Dr. Stockman stated he completed the training and
will submit he certificate soon. Joni Courtney must also submit her certificate as soon as
possible.

The Elgin Board of Health quarterly meeting was held on Monday, April 8, 2013 in the
Human Resources Purchasing Conference room at City Hall. The meeting was called to
order by Dr. Poulos at 7:33 p.m.
Members Present: Dr. Franz Stockman, Dr. Dorothea Poulos, Cynthia

Approval of Minutes: Dr. Poulos moved to approve the minutes from the January 28,
2013 Board of Health quarterly meeting as submitted. Dr. Franz Stockman seconded the
motion. The motion was adopted by a vote of four yes, zero no, and one abstention.
REVIEW OF THE FIRST QUARTER OF 2013
In the first quarter of 2013, Portillo's and Edible Arrangements opened as food handling
establishments. Mudd and Eddies, McLean Mart, Cary's Bakery, Mangos Cafe y
Neveria. Goodfellas, Bubba McDuffy's, San Luis Bakery. and Erencia De Mis Padres
closed.

De La Rosa Market closed and reopened as La Esperanza #2. La Rosita Bakery closed
and reopened as Le Paris Bakery. Elgin Books moved from East Chicago Street to South
Grove Avenue. In the El Mesias Mall, Fruteria Guzman closed and reopened as Petra
Maldonado, and in the Fox Valley Flea Market, Camaroncito Loco closed and reopened
as Luiz Produce. Salon Couture no longer is licensed to sell alcohol.
Approximately 21 pounds of food from Elgin Shell at the Grove was discarded due to the
absence of hot water at the facility for hand and utensil washing. About two pounds of
food was discarded from Elgin Buffet due to temperature violations. Roughly 33 pounds
of food was discarded from Lucky Asian Market as the food was from an unapproved
source. El Cokito was fined $500 in adjudication for allowing customer dining on their
premises without restroom available to the public.
Dino's Foods had a fire that started in the deli that spread smoke throughout the entire
establishment. This was one year and three days after the store reopened from a previous
fire. The store was closed and remains closed, and nearly the entire food inventory was
discarded.

MISCELLANEOUS
The Kane County Health Department reviewed the food program for compliance with the
Illinois Department of Public Health Local Health Protection Grant Rules. Kane County
stated they will provide the results of this review this month.
Vector Disease Control International will provide the mosquito control for the Kane
County portion of Elgin this season. Clarke had previously provided this service for
Elgin since at least 1982. Mr. Bohner has met with representatives from VDCI twice to
discuss the methods of mosquito control and surveillance that they will provide.

July 8,2013
The Elgin Board of Health quarterly meeting was held on Monday, July 8, 2013 in the
Community Development conference room at City Hall. The meeting was called to order
by Dr. Poulos at 7:34 p.m.
Members Present: Dr. Dorothea Poulos, Cynthia Applehoff-Siedsma, Jeanine
Seitz-Partridge, Joni Courtney, Edward Whitcomb
Members Absent: Dr. Franz Stockman
Staff Present: Brad Bohner, Public Health Coordinator
Approval of Minutes: Dr. Poulos moved to approve the minutes from the April 8, 2013
Board of Health quarterly meeting as submitted. Cynthia Applehoff-Siedsma seconded
the motion. The motion was adopted by a vote of four yes, zero no, and one abstention.
REVIEW OF THE SECOND QUARTER OF 2013
In the second quarter of 2013, Roque's Mexican Grill, Paleteria Y Neveria La
Michoacana on South McLean Boulevard and Lou Malnati's Pizzeria opened as food
handling establishments. The Salvation Army on Douglas Street was licensed as a food
pantry. Caribou Coffee, Dino's Food and The Tilted Kilt closed.
The Grand Victoria food court reopened after three months of remodeling. Club 58
closed and reopened as Fatsoz. Dulceria Yanelly on South Street closed and reopened as
Antojitos Y Dulceria La Mexicana. In the Neighborhood Deli closed for 5 days due to an
electrical surge that led to a loss of power. Mr. Bohner attended adjudications hearings
for La Quebrada for leaking effluent from their dumpster onto a public sidewalk, La Casa
de Pollo Asado for dumping grease onto a neighboring parking lot, and Charles Fast
Food #5 for frying food without an approved grease trap, hood, and fire suppression
system.
The Comfort Inn and Suites spa, the Holiday Inn spa, and the Dell Web spa were closed
for one day due to improper disinfectant levels. The Hampton Inn spa was closed for
three days due to high disinfectant levels. The Lexington Inn and Suite pool and spa were
closed for 13 days due to unsafe conditions due to construction in the swimming facility
area.
During June Mr. Bohner noted that the Health Division conducted 41 seasonal and
temporary food inspections mostly at the farmers' markets in Elgin. In addition, the
health department inspected every vacant property that obtained a pool permit in the last
20 years to check for possible stagnant water.

MISCELLANEOUS
Mr. Bohner explained that Chapter 19 of the Elgin Municipal was amended to correlate
with the Illinois Department of Public Health Cottage Food Law. This will allow Elgin
residents to prepare non-potentially hazardous food in their home for sale at farmers'
markets. The cottage food operator must have an IDPH Food Service Sanitation
Manager Certificate and register with the City of Elgin before selling food.
Mr. Bohner stated that Vector Disease International continues with surveillance of larval
and adult mosquitoes in the Kane county portion of Elgin. The heavy rainfall this spring
and summer will increase the number of floodwater mosquitoes but reduce the number of
Culex mosquitoes that carry West Nile Virus. VDCI will complete their treatment of
catch basins with mosquito larvacide by the end of July. The Northwest Mosquito
Abatement District is completing the same work in the Cook County portion of Elgin.

October 14, 2013
The Elgin Board of Health quarterly meeting was held on Monday, October 14, 2013 in
the Community Development conference room at City Hall. The meeting was called to
order by Dr. Poulos at 7:28 p.m.
Members Present: Dr. Dorothea Poulos, Cynthia Applehoff-Siedsma, Jeanine
Seitz-Partridge, Joni Courtney,
Members Absent: Dr. Franz Stockman, Edward Whitcomb

Approval of Minutes: Dr. Poulos moved to approve the minutes from the July 8, 2013
Board of Health quarterly meeting as submitted. Cynthia Applehoff-Siedsma seconded
the motion. The motion was adopted by a vote of four yes, and zero no.
rik REVIEW OF THE THIRD QUARTER OF 2013
In the third quarter of this year, the Eastside Recreation Center concession stand, the
Circle K gas station on South McLean Boulevard, and the Larkin Café opened. Jerry's
Café, a coffee shop in Judson University's Harm A. Weber Academic Center was also
licensed after obtaining the proper equipment and permits. Triple A Catering licensed a
mobile food truck.
McLean Shell, Veggie Place, Edison Shell, Villa Verone, Petra Maldanado, and Le Paris
Bakery #3 closed. The cafeteria in Weiler Engineering also closed. The owner
Camaroncito Loco was fined $1,000.00 for operating an unlicensed mobile food truck,
which is now properly licensed. The food handling license for El Nueve Rincon de
Pastigel Bakery was suspended for four days as the establishment was substantially
remodeled without plans or permits, and for using fryers and other gas appliances without
a hood or fire suppression.
The Health Department discarded approximately 22 pounds of food from Petra
Maldanado, 20 pounds of ground beef from Taqueria El Paraiso on South McLean
Boulevard, 5 pounds of tripe from Taqueria Los Comales, 8 pounds of alfredo sauce from
Jimmy's Charhouse, and 4 pounds of food from Circle K on South McLean Boulevard
due to temperature violations.

The Health Division has conducted 102 seasonal and temporary food inspections so far
this year, an increase from last year due to the increase of food vendors at the farmers'
markets and summer food program sites in Elgin. In addition, the health department
inspected every vacant property that obtained a pool permit in the last 20 years to check
for possible stagnant water, and conducted 70 inspections of standing water and stagnant
swimming pool complaints. The mosquito season ended this year without a West Nile
Virus case reported in Kane County to date.
MISCELLANEOUS
Dr. Poulos stated that she was interested in expanding the scope of the Board of Health to
address issues other than the Environmental Health programs currently in place. Mayor
Kaptain stated it is within the purview of the Board of Health to make any
recommendations it finds necessary to promote the health for the citizens of Elgin, as
exemplified by the smoking ban the Board pursued several years ago.
After much discussion, Dr. Poulos mentioned that childhood obesity and nutritional
education are issues that the Board may wish to address to enhance the health of the
citizen of Elgin. Dr. Poulos stated that she is a member of the Kane County Board of
Health Advisory Committee and she will mention at their next meeting that the Elgin
Board of Health may be interested in partnering with them in some capacity. Cynthia
Applehoff-Siedsma, Jeanine Seitz-Partridge and Joni Courtney each mentioned that
Advocate Sherman Hospital, Presence St. Joseph Hospital, and Delnor Hospital all offer
classes and host wellness fairs that deal with these issues. Also, the Kane County Health
Department has many resources that deal specifically with these issues. Furthermore,
Mayor Kaptain indicated that the City of Elgin Parks and Recreation Department is
continuously looking to reach out to the community to increase resident's participation in
exercise and nutritional programs.
After much discussion Dr. Poulos made a statement that "The City of Elgin Board of
Health would like to focus on personal health in the community. While many topics are
valid, the Board would like to first focus on childhood obesity, poor nutrition, and lack of
physical activity." Joni Courtney made a motion to accept this statement as a goal for the
Board of Health. Jeanine Seitz-Partridge seconded the motion. The motion passed 4 yes
O no.
As a first step, Dr. Poulos encouraged Board members to forward links to websites with
these goals to the Public Health Coordinator who can post them on the City of Elgin
website under the Board of Health heading. In addition, the Board of Health will contact
Wayne Carlstedt, Associate Manager of Operations at the Centre, to discuss their desire
to help sponsor children's' activities at the Centre during the Martin Luther King
Holiday.
