The meeting was called to order by Chairman Hirsch at 7:24 p.m. Brad Bohner introduced the Health Department intern for the winter of 2011, Ms. Laura Squatrito. The minutes from the October 11, 2010 meeting were approved. REVIEW OF THE FOURTH QUARTER The following food handling establishments opened in the fourth quarter of 2011: New Taste, E's Hanger, La Cabana, Provena St. Joseph Coffee Shop, Fox Valley Discount Restaurant, and Extra Value wine and Spirits. New Tortilleria opened and closed within one month. The following restaurants and retail food stores also closed: Bowes Retirement Center, Deli Three, Thai Town Restaurant, Good Shepherd School cafeteria, Taqueria Parilla Azteca, Dunkin Donuts on South McLean, The Hangout, Barefoot Café, American Little League concession stand, Susie's Mobile Company, and Steam'n Joe Junction. The sanitarian discarded food from the following establishments for spoilage or temperature abuse: 5 pounds from Boston Market, 15 pounds from Colonial Ice Cream, 5 pounds from Golden Chinese Buffet, 3 pounds from The Greens of Elgin, 30 pounds at Sherman Hospital, 13 pounds at Elgin Community College Retail Food Store, 10 pounds at La Roca, 30 pounds at Green Jade, 20 pounds at Quiroz La Rosita, 37 pounds at Butera, 1 pound at Carmaroncito Loco, 35 pounds at El Faro on State Street, and 8 pounds at Joseph's/Bennigans. r Page Two Minutes January 10, 2011 The court case at La Quebrada Restaurant for grease leaking onto a public sidewalk was complied. However, they were closed for 24 hours due to flooding, fire, an insect infestation, and other sanitary issues. Armando' and Sons was closed for 4 hours, and Papa John's Pizza was closed for 5 hours due to no available hot water. The Sanitarian and Public Health Coordinator were able to attend a Bed Bug Solutions Symposium. The City has received several bedbug complaints from apartment tenants. Once confirmed, the owner of the dwelling is required to hire a certified pest control company to eliminate these insects, which is often expensive and time consuming. A standard operating procedure for these complaints was developed by the Code Enforcement manager and the Public Health Coordinator. Dr. Stockman asked if the flea markets in town were selling food. Mr. Bohner stated that all three operating flea markets have restaurants, as well as food booths with limited food preparation. All food booths that handle food must have at minimum a handsink and convenient access to an onsite three compartment. In addition, all barber and beauty shops, massage parlors, tattoo parlors, and body piercing booths must have permanent handsinks as well as a license from the Illinois Department of Public Health. The meeting was called to order by Chairman Hirsch at 7:30 p.m. Members Present: Dr. Ronald Hirsch, Edward Whitcomb, Joni Courtney, Jeanine Seitz-Partridge Members Absent: Dr. Franz Stockman Staff Present: Brad Bohner, Public Health Coordinator Approval of Minutes: Dr. Hirsch moved to approve the minutes from the January 10, 2011 Elgin Board of Health quarterly meeting as submitted. Ms. Courtney seconded the motion. The motion was adopted by a vote of four yes, and zero no. REVIEW OF THE FIRST QUARTER OF 2011 The Public Health division licensed the following food handling establishments in the first quarter of 2011. Dulceria Yanelly, Cocteleria Martinez, Fruteria Guzman, and Paleteria Juanitas opened in the El Mesias Mall. Carmaroncito Loco opened in the Elgin Flea Market. Also opening were Mango Café and Neveria, and Babylon Kebob. The Evangelical Free Church licensed their kitchen as a soup kettle, and the Hemmens Cultural Center kitchen was licensed. Coliseum Pizza closed and reopened as Arvatis Pizza. The following restaurants and retail food stores closed: Golden Chinese Buffet, Elgin Clark, Dulceria Yanelly on Liberty Street, Porter's Pub, Quiznos, Quiroz La Rosita, and El Chicanito. Carnival Sweets, Treats and Specialties, Elgin Parent's Coop, and 2 J's Wine and Spirits no longer sell potentially hazardous food. The Public Health Division closed Woodview Lanes for 24 hours due to no hot water available in the facility. In addition, their snack shop was closed for nine days since they did not have a three compaitment sink to wash, rinse and sanitize utensils. La Casita was closed for four hours due to no heat available in the establishment. Hot Spot at Home Depot was closed and remains closed due to non-payment of their food handling permit. La Victoria was closed and remains closed due to a fire. Carmaroncito Loco was closed for 24 hours due to non-payment of their food handling permit, and was later closed for four hours due to no hot water available in the establishment. - Page Two Minutes of Meeting April 11, 2011 The Public Health Division discarded food from the following establishments for spoilage or temperature abuse: 35 pounds form Gayatri Mini Mart, 3 pounds from Big Timber Marathon, 30 pounds from the Elgin Riverside Club, 24 pounds from Dunkin Donuts on East Chicago Street, 5 pounds from El Cokito, 10 pounds form El Faro III, 25 pounds from Taqueria Chapala, 5 pounds from Birrerra La Provincia, 15 pounds form Chorizo Grill, and 3 pounds from Judson University cafeteria. Stop work orders were placed at Dunkin Donuts at Clock Tower Plaza and Cary's Bakery for working without permits. The following spas were closed by the Public Health Division for 24 hours due to improper disinfectant levels: The Centre men's and women's spas, Country Inn and Suites, Hampton Inn, and Comfort Suites. The Day's Inn/Elgin Inn spa was closed for 24 hours on two occasions, and the Holiday Inn spa was closed for 24 hours on three occasions. The Centre therapy pool was closed for 24 hours due to an inoperable filter, and both of The Centre's spas were closed in February for 12 days due to high bacteria growth levels in water samples. SMOKE FREE ILLINOIS ACT Dr Hirsch discussed the recent activity to remove the restriction on smoking in casinos. He reviewed the activities of the Board in the past related to enacting a smoking ban in the City of Elgin prior to enactment of the state-wide ban. Dr. Hirsch moved that "the Board of Health of the City of Elgin opposes the removal of the smoking ban in casinos in order to protect the health of the residents of Elgin and other communities who work at and patronize those establishments." Ms. Courtney seconded the motion. The motion was adopted by a vote of four yes and zero no. Adjournment: There being no further items to discuss, Ms. Seitz-Partridge moved to adjourn the meeting at 8:10 p.m. Mr. Whitcomb seconded the motion. The motion passed four yes and zero no. The meeting began at 7:30 p.m. Members Present: Dr. Ronald Hirsch, Edward Whitcomb, Joni Courtney, Members Absent: Dr. Franz Stockman, Jeanine Seitz-Partridge Staff Present: Brad Bohner, Public Health Coordinator REVIEW OF THE SECOND QUARTER OF 2011 Cary's Bakery opened in Dino's, and Ramiro's Restaurant, and Canoas Los Mariscos opened in El Mesias Mall. China King, Joe's BBQ and Fish, and HotFast Pizza also opened. The Centre of Elgin kitchen and Porter's Pub were licensed as food handling establishments. Big Apple Bagel relocated within the Wing Park Shopping Center. During this quarter Streets of Chicago, Pastigel Bakery in Armando's, and Cocteleria Martinez in El Mesias Mall closed. The Public Health Division discarded food from the following establishments for temperature violations: 116 pounds from White Cottage Pizza, 9 pounds from Dairy Queen on Villa Street, .12 pounds from Target, 14 pounds from Jester's, 14 pounds from Wendy's on South McLean, 4 pounds from Dulce Vida, 105 pounds from Carmina's, 10 pounds from Taqueria Los Comales, 3 pounds from Burnt Toast, 3 pounds from El Faro, and 7 pounds from Taqeria El Paraiso. Two unlicensed food booths at the El Mesias Mall were closed. The Public Health Coordinator attended an adjudication hearing for Delicia Tropical Café for installing plumbing without permits, and attended a court hearing for El Rincon de Pastigel for numerous violations. The Illinois Department of Public Health trained the Public Health Coordinator and sanitarian to obtain certificates from the Illinois Department of Agriculture to apply solid mosquito larvacide. In addition, the Health Department presented food worker training sessions for several people on two occasions. The Health Department closed Hunter's Ridge swimming pool for two days, Fox River Country Day School pool for one day Hampton Inn pool and spa for two hours, and Country Inn and Suites spa for one hour due to low disinfectant levels. Tyler Tower swimming pool was closed for one day due to an inoperable water pump. The Original JJ Chicken and Fish, Red Bar Winery, Birreria La Provencia, Woodview Lanes, La Puesta Del Sol, and Elgin Firepit closed. Elgin Community College Student Resource Center opened after a remodel, and Family Dollar relocated within Wing Park Shopping Center. Camaroncito Loco was closed for three days since no hot water was available to the establishment. Dino's and Cary's Bakery were closed due to a fire and have not yet reopened. The Public Health Division discarded food from the following establishments for temperature violations: 300 pounds from the Grand Victoria Casino Buffet, 3 pounds from the Elgin Firepit, 2 pounds from Sherman Hospital, 177 pounds from Pauls' Family Restaurant, 212 pounds from Trefon's Family Restaurant, and 3 pounds from De La Rosa Market. Approximately 330 pounds of food was discarded from Sherman Hospital due to a sprinkler head discharge in the cafeteria. The Elgin Fresh Market was closed for 4 days and approximately 185 pounds of food was discarded due to a possible food borne illness441, outbreak. However, an ongoing foodborne illness outbreak was not definitively linked to this establishment. The Public Health Coordinator attended an adjudication hearing for La Quebrada for allowing grease to accumulate on a public sidewalk. A food worker training session was conducted for several food service employees at their work site. The Health Department closed Hunter's Ridge swimming pool for four days, Fox River Country Day School swimming pool for three hours, Hampton Inn swimming pool for one hour, Garden Quarter Townhomes swimming pool for one day, Festival Park water feature for one day, and Hunter's Ridge Swimming pool for four days due to low disinfectant levels.
