ELGIN BOARD OF HEALTH
January 7, 2002

The Elgin Board of Health regular meeting was held on Monday, January 7, 2002 in the second
floor conference room in the north building at City Hall. The meeting was called to order by
Chairman Hirsch at 7:33 p.m.
APPROVAL OF MINUTES
Ms. Demuth made a motion to approve the minutes of the October 22, 2002 meeting, seconded
by Mr. Stuart. Motion carried on a vote of 5 Yes and 0 No.
4th QUARTER REPORT
The following food service establishments opened during the fourth quarter; Mi Tierra Tacos Y
Mas on Larkin Avenue in the old Checkers Restaurant location, Dunkin Donuts/Baskin Robbins
on East Chicago Street in the former Butternut Bread retail store, Vance Grocery relocated from
Raymond Street to 1118 South Street, Walmart is now licensed to sell as retail food store, Wing
Park Golf Course Clubhouse will sell prepackaged sandwiches, TNT Hotdogs inside the Home
Depot on Randall Road, Winston Puris an ice cream vendor at the State Street Market, Sam's
Pizza on Randall Road which has since closed and re-opened as Tortorice's Pizza, Café
Magdalena on Douglas Avenue and Sonia's Pump N' Mini-Mart on Summit Street.
The following licensed food service operations closed in the fourth quarter; Mother Goose
Daycare on Standish Street and The Coffee Shop on South McLean Boulevard.

Plan reviews were conducted for Beef Villa on Larkin Avenue that was demolished and will be
rebuilt with a drive-thru lane, Elgin Community College day care and B P Amoco on Dundee
Avenue for an interior remodel.
REVIEW YEAR END REPORTS
The year end reports were briefly discussed. Mr. Slaby answered questions about food borne
illnesses and what constitutes a valid food borne illness incidence, as well as the number of"high
risk"initial inspections and reinspections.

Dr. Hirsch asked for an update on Mr. Slaby's request for additional staff and an upgrade for Mr.
Bohner. Mr. Slaby stated that the request for additional staff was not approved and he will need
to present it again in 12 to 18 months. The Sanitarian upgrade was sent to Human Resources for
their review.
There being no further business to discuss, the meeting was adjourned at 8:18 p.m. The next
meeting will be on Monday, April 15, 2002.

The Elgin Board of Health regular meeting was held on Monday, April 15, 2002 in the second
floor conference room in the north building at City Hall. The meeting was called to order by
Chairman Hirsch at 7:30 p.m.
APPROVAL OF MINUTES
Mr. Stuart made a motion to approve the minutes of the January 7, 2002 meeting, seconded by
Dr. Stockman. Motion carried on a vote of 5 Yes and 0 No.
REVIEW 1ST QUARTER REPORTS
The following food service establishments were opened during the first quarter of 2002; McLean
Shell, Motorola cafeteria, BP Amoco (remodel), Jewel (Larkin Avenue remodel), Weiler
Engineering cafeteria. Mr. Slaby reported the following food service operations closed during
the first quarter of 2002; Asian Dragon House, Weiner Wagon, Baskin Robbins (Summit St),
Rancher's Barbeque, and Leitners.
Mr. Slaby stated that his office had discarded 100 containers of food from an unapproved source
at Vance Grocery and 118 containers at Lao Trading company for the same reason.
The Walmart has been approved to sell frozen foods like pizza and ice cream.
MISCELLANEOUS
Mr. Slaby's office is working on an"odor ordinance"and will probably have a draft at the July
meeting.

Clarke Outdoor Spraying Company has the contract for mosquito abatement in Elgin. Their
abatement program will begin soon.

Chairman Hirsch at 7:30 p.m.
APPROVAL OF MINUTES
Mr. Whitcomb made a motion to approve the minutes of the meeting of April 15, 2002, seconded
by Mr. Stuart. Motion carried on a vote of 5 Yes and 0 No.
REVIEW 2ND QUARTER
The following food service operations opened during the second quarter; Jetsonic (formerly
Amoco) on South McLean and Main Lane. Mr. Slaby noted that they will have a larger mini-
mart. Subway on Clock Tower Plaza, Beef Villa on Larkin Avenue, Unique Mart on North
McLean Boulevard, Smiley=s Beef and Gyros on Larkin Avenue and Mi Tierra Tacos Y Mas on
Highland Avenue (formerly Weiner Wagon).
The following food service operations closed during the second quarter; the cafeteria at Sherman
Resources at 1019 East Chicago Street, the Casino Cinema on South Grove Avenue, 212 Club on
e"` Ball Street and Big Lots on Airport Road.

A number of pools were closed in the second quarter, however the Days Inn pool was closed for
ten days. Mr. Bohner explained that he initially closed the pool and since the paint was peeling
from the bottom of the pool he suggested that it was a good time to have the pool repainted. The
pool was drained,painted and refilled. The whole process took ten days.
Mr. Bohner stated that the Illinois Department of Public Health has requested that he conduct
their swimming pool inspections for locations in Elgin. This would involve only about six (6)
pool sites and the initial inspection would take about one-half hour longer than his normal
inspection.
MISCELLANEOUS
Mr. Slaby is working on an odor nuisance ordinance and provided copies of a sample ordinance
from Linn County, that he has submitted to the Legal Department for review. After a brief
rkdiscussion, Mr. Slaby said he would keep the Board informed of his progress.
The Health Division is also working with Print Pack regarding complaints from neighbors about
noxious odors from their operation. Mr. Slaby said he will be meeting with the people from Print
Pack and states that his goal is to make sure that emissions from their company are not a public
health hazard. He will encourage them to be a Agood@ neighbor in attempting to resolve this
issue.

The budget process is starting and Mr. Slaby stated that he will try to get another Sanitarian and
an upgrade to management for the Sanitarian positions.

The next meeting is scheduled for Monday, October 7, 2002. There being no further business to
discuss, the meeting was adjourned at 7:50 p.m.

floor conference room in the north building at City Hall. The meeting was called to order by
Chairman Hirsch at 7:30 p.m.
APPROVAL OF MINUTES
Ms. Ward made a motion to approve the minutes of the July 15, 2002 meeting, seconded by Mr.
Stuart. Motion was carried on a vote of 5 Yes and 0 No.
REVIEW 3RD QUARTER
Mr. Slaby reported the following food service establishments that opened during the third
quarter; Elliot=s Off Broadway, Pars Ice Cream, J &K Foodmart, Otter Creek Elementary
School, Bobby D=s Pizza, Village Green of Elgin, Latte Dah Café, Subway Sandwich
Restaurant, Angelo=s and Gifford High School Cafeteria. There were no permanent food service
closings in the third quarter, however, one license was suspended for six (6) hours while
corrections were being made to an inoperable sewer line.

MOSQUITO CONTROL MEASURES
Mr. Slaby stated that the number of inspection for food service are down because of the number
of calls his office has been handing regarding birds and the West Nile Virus. There was an
increase in the number of pool inspections and complaints about stagnant water as a result of the
virus. Mr. Slaby and the Board briefly discussed the letters that had been received, requesting the
City spray for adult mosquitos in the wake of the West Nile Virus.
MISCELLANEOUS
Mr. Slaby stated that a decision package had been presented for a new ASanitarian@ position
eft. within the department. This person will probably have to do Alead risk@ inspections. Also, he
had attended a brainstorming session for the proposed PADS location on South State Street,
south of the Elgin Mental Health Center.
There being no further business to discuss, the meeting was adjourned at 7:58 p.m.
