CHANGE ORDER NO. 1 AND FINAL FOR BID ITE C-5
SCOPE: Change Order No. 1 and Final will add fourteen ork items to and delete three work
item from the lump sum contract with Lamp, Inc. for Bid I em C-5, "Carpentry" of the New
Public Works Facility Project- Bid Package C.
REASONS FOR CHANGE: This Change Order No. 1 . d Final is in the best interest of the
City and is authorized by law. The construction work sco.- adjustments were unforeseen at the
time of original contract award.
SUMMARY OF CHANGES IN CONTRACT AMO 'T: The amount the contract will be
revised is as follows:

order not corrected after security plan changed.
17. Provide and install louvers on door to electrical panel ro s m. $204.00
18. Deduction for interior signage due to unsatisfactory performance of -$4,800.00
sub-contractor.
The original contract with Lamp, Inc. is for $302,475.00. B reason of Change Order No. 1 and
Final, the contract for Bid Item C-5, "Carpentry" of the Ne Public Works Facility Project - Bid
Package C will be decreased by the amount of six thousand, i ne hundred and nineteen and 66/100
dollars(-$6,119.66). The new contract amount, inclusive of Change Order No. 1 and Final, is
two hundred and ninety-six thousand, three hundred and 1 ' -five and 34/100 dollars
($296,355.34).
OTHER CONTRACT PROVISIONS: All other contract provisions shall remain the same.
