RESOLUTION
AUTHORIZING RENEWAL OF GROUP HEALTH INSURANCE
WITH TRUSTMARK INSURANCE COMPANY
BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF ELGIN,
ILLINOIS, that Robert O. Malm, Interim City Manager, be and is
hereby authorized and directed to renew the City' s stop loss
(excess coverage) for its group health insurance with
Trustmark Insurance Company pursuant to the summary of the
Trustmark Insurance Company renewal proposal, a copy of which
is attached hereto and made a part hereof by reference.

SUBJECT: Group Health Insurance Renewal
PURPOSE
The purpose of this memorandum is to provide the Mayor and
Members of the City Council with information to consider
renewal of the City' s Health Maintenance Organization (HMO)
and Self Insurance Plans .
BACKGROUND
rft. The City has two group health insurance plan options avail-
able for all full-time employees . A fully self insured
indemnity plan and an HMO plan through HMO Illinois (Blue
Cross Blue Shield) . Specific and Aggregate Insurance are
provided for the Self Insurance plan by Trustmark Insurance
Company.
Bid specifications were sent out by our broker, Robert L.
Schmidtke and Associates, to seventeen carriers for the
stop-loss (excess insurance) coverage and to six HMO provid-
ers . After reviewing submitted quotes (attachment 1) , it was
recommended that it will be in the City' s best interest to
renew with our current carriers for the 1997/98 year. A
letter (attachment 2) from Blue Cross/Blue Shield indicating
that they can not compete with our current rate for excess
coverage and administrative cost is attached for your informa-
tion.
Our overall premium increase for the HMO plan will be 5% and
the Specific Stop Loss renewal increase will be 5 . 9% . The
increase in the specific premium is due to the fact that we
had a claim that exceeded our $100, 000 specific attachment
point and four that were over 50% of the attachment point in
1996/97 . Administration and Preferred Provider Organization
cost will remain the same as in 1996/97 .

RECOMMENDATION
it is recommended that the City renew its HMO Plan with HMO
Illinois and our excess coverage with Trustmark Insurance
Company.

• Stop Loss Renewal
Based on competitive bidding,Trustmark Insurance Company represented the lowest maximum
cost of those carriers solicited for bids. Trustmarks renewal represents an increase in maximum
annual cost of less than 1%over the prior year. Based on these facts we would recommend
accepting the Trustmark renewal.
• HMO Renewal
Based on competitive bidding, HMO Illinois(Blue Cross)represented the lowest cost of HMO's
solicited for bids. The final rates represented an increase of 5%. Therefore we would
recommend accepting their renewal at this time.

Dear Bob:
After a thorough review by our underwriters of your proposal request for the City of
Elgin,we regrettably will be unable to provide you with a proposal. Our intention was to
provide you with a PPO program representing 100%in-network and 80%out-of-network
hospital benefit levels. The resulting underwriting analysis was an uncompetitive proposal
as compared to their current aggregate claims liability, specific and aggregate premiums
and administrative costs.
We certainly appreciate your submission of the proposal request to us. Additionally,we
appreciate the opportunity to continue to provide HMO coverage to the employees of the
City of Elgin.
