NOW, THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF ELGIN, ILLINOIS, that Richard B. ELGIN, ILLINOIS BIG TIMBER/TYLER CREEK INTERCEPTOR CHANGE ORDER NO. 6 PAGE 1 SCOPE: This Change Order No. 6 covers additional changes in the scope of work at the Big Timber/Tyler Creek Interceptor that was not reasonably foreseeable at the time the contract was signed and are in the best interest of the City. The items covered in this change order are for rip-rap and hydroseeding along Tyler Creek. This contract for construction is with Szabo Contracting, Elmhurst, Illinois. REASONS FOR CHANGE: Item 1. Invoice #9204X25: This project was designed and bid locating the 36-inch diameter sanitary sewer approximately 100 feet north of Tyler Creek. It was determined that the project fell within the guidelines of the U.S. Army Corps of Engineers Nationwide Permit. However, after a USACOE site inspection, they requested the installation of rip-rap material, where a drainage swale crosses the interceptor and empties into Tyler Creek, at approximate Station 78+00. This will result in a net addition to the contract. Invoice #9204X26: This project was designed and bid with no surface restoration being performed by the contractor. The intent was to final grade the area disturbed by construction and allow the natural vegetative growth to reestablish itself. After a U.S. Army Corps of Engineers inspection, they determined that the area was to be restored immediately by hydroseeding. This will result in a net addition to the contract. SUMMARY OF CHANGES IN CONTRACT AMOUNT The amount of the contract will be revised as follows: Item 1. Add 30 tons of rip-rap, installed complete. 30 tons X $40.00/ton = $1,200.00 Total Add = $1,200.00 Clark Dietz.Inc.
