FOR THE LORD'S PARK PAVILION WHEREAS, the City of Elgin has heretofore entered into a contract with Elgin Builders, Inc. for renovation of the Lord's Park Pavilion; and WHEREAS, it is necessary and desirable to modify the terms of the contract as is described in Change Order No. 6, attached hereto. NOW, THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF ELGIN, ILLINOIS, that Larry IT. The Contractor for this project is Elgin Builders, Inc. , of Elgin, Illinois. REASONS FOR CHANGE: This Change Order is required for the costs associated with the items requested by staff to complete the following work. 1. Additional hardware equipment was required to be substituted to complete the full operation of the doors. This circumstance was unforeseen as the original specified sliding door hardware was not available anymore due to the company closing its manufacturing plant. A substitute manufacturer had to be found. 2 . Replacement of porch deck in the south east corner. The deck in the area was found to be soft and spongy due to a weakened subfloor below the deck. This bad section of decking was hidden and not able to be seen. 3. Porch deck replacement under columns (two separate occasions) . These unforeseen field condition was discovered after placement of the new columns. The subdeck under the existing deck had rotted and was in need of replacement. 4. Level floor in the round warming room. The floor in the warming room had sloped towards the center of an abandoned drain. The City requested the floor be leveled to facilitate floor tile installation. 5. Install fan relay shut down switches. The installation of the relay switches was to be performed under a separate contract by a different Contractor who was unable to perform these services. This portion of the work was then added to the Scope of Work under this Contract. This change is germane to the Contract and required for occupancy of the building. 6. Replacement of cracked stained glass. Under the Base Bid all obvious stained glass panels that were cracked or missing were to be replaced. These additional panels were found to be cracked and not seen in the original survey, due to their inaccessible locations. 7. Replace the sprinkler system flow switch. The original flow switch that was specified was not compatible with the fire alarm system installed under a separate Contract by a different Contractor. This change is essential for the performance of both the sprinkler and fire systems. 8. Additional work on round roof. The original proposed change in construction had been revised after an on site meeting. However, the original proposal had been submitted (AWA #39 - $3,846.00) in lieu of the revised proposal (AWA #39R - $4,338.00) for Change Order #4. The difference in the amount is $542.00. The new revised contract total will be One Million Eighty Six Thousand Five Hundred Twenty Three and 00/100 Dollars ($1,086,523 .00) . Rice, City Manager SUBJECT: Change Order No. 6 and Final for Remodeling Work at Lords Park Pavilion PURPOSE The purpose of this memorandum is to forward to the Mayor and City Council Change Order No. 6 and Final for the remodeling work at the Lords Park Pavilion. elk BACKGROUND Architects Dahlquist and Lutzow prepared plans and speci- fications for the remodeling of the Pavilion. Subsequently, a contract was awarded to Elgin Builders for the construction which was begun on September 9, 1991. The rehabilitation work on the building is now substantially completed. Attached is Change Order No. 6 and Final from Dahlquist and Lutzow regarding the remodeling work. The change order includes eight items with a total net cost of $8,004 .00. The architect will be present at the meeting of February 10th to answer any questions you may have regarding this change order. FINANCIAL IMPACT The change recommended by the architects will result in an $8,004.00 increase in the contract with Elgin Builders from $1,078,519. 00 to $1,086,523. 00.
